- Solicitation Attachments 1-19 Solicitation (RFQ) (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (ii) This reference number for this effort is W912ES-15-T-0079 and the solicitation is issued as a request for quotation (RFQ). (iii) The solicitation document and incorporated provisions are those in effect through Federal Acquisition Circular 2005-81, effective 10 April 2015. (iv)This solicitation is issued as a total small business set-aside. (v) Contract Line Items: See "W912ES-T-0079 Request for Quotation SF 1449" as posted on the Federal Business Opportunities website (www.fbo.gov). (vi) Description of requirements for items to be acquired: The work includes application of herbicides to control undesirable vegetation at eight (8) forest management locations on Corps of Engineers property totaling up to approximately 97.86 acres in Pierce and La Crosse Counties in Wisconsin, and Wabasha and Goodhue County in Minnesota. Undesirable vegetation is any vegetation that competes with regenerating forest trees for growing space, water and nutrients. Multiple applications using a variety of herbicide products will likely be needed during 2015 to achieve vegetation control goals extending into the 2016 growing season. All areas are not accessible by land (boat access required). Vicinity maps are in Attachments 1-8. Site specific maps are in Attachments 9- 18. (viii) The provision at 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. 52.212-1 Addendum - Notes To Quoters a. QUOTATIONS MUST INCLUDE THE FOLLOWING: (1) Complete Standard Form (SF) 1449 with acknowledgement of Amendments (2) Completed Pricing Schedule (3) Applicable State Licenses in accordance with paragraph 1.6.3 of the scope of work b. Quotations may be sent by email (preferred) or by mail to: U.S. Army Corps of Engineers - St. Paul District Attn: Shamron Richardson, Contract Specialist 180 East 5th Street, Suite 700 Saint Paul, MN 55101-1678 Shamron.j.richardson@usace.army.mil c. Quoters are urged and expected to inspect the site where the work will be performed. Site visits may be arranged during normal duty hours by contacting Mr. Bobby Jackson at (651) 290-5899 or by email at bobby.g.jackson@usace.army.mil. d. Offerors must be registered in the System for Award Management (SAM), including online representations and certification. Please see www.sam.gov for registration information. f. In accordance with Federal Acquisition Regulation (FAR) Subpart 9.1, a prospective contractor may be requested to provide additional information in order to assist the Government in making a responsibility determination. g. Quotations must include a price for all items in this solicitation to be eligible for award. The Government will award on an "All or None" basis. Evaluation of quotations will be based, among other factors, upon the total price included on the pricing schedule for all items. h. Offerors are advised that the U.S. Army Corps of Engineers as an agency of the Federal Government is tax exempt (Tax ID# 62-1642142). (ix) The provision at 52.212-2, Evaluation - Commercial Items, applies. The following factor shall be used to evaluate offers: Price. (x) The provision 52.212-3, Offeror Representations and Certifications - Commercial Items (Alt 1) applies. Offerors must complete online representations and certifications in the System for Awards Management (SAM) at www.sam.gov.
