SYNOPSIS/SOLICITATION ISSUED April 24, 2015 PART: U.S. Government Procurements SUBPART: Supplies, Equipment, and Materials CLASS CODE: 71- Furniture OFFADD: Bath VA Medical Center, 76 Veterans Avenue, Bath, NY 14810 SUBJECT: Cafeteria / Dining Room Furniture - Tables and Chairs SOL: VA528-15-Q-0175 DUE: May 1, 2015 POINTS OF CONTACT: Edward Lyke, edward.lyke@va.gov WEB ADDRESS: http://vaww.visn2.med.va.gov/ DESCRIPTION: This is a combined synopsis solicitation for a commercial item prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented by additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. Proposals are being requested for Request for Proposal (RFQ), VA528-15-Q-0175 for the following: Receipt and delivery of specified cafeteria / dining room furniture for a VA Medical Center (see attached SOW) Solicitation VA528-15-Q-0175 is issued by VA Medical Center, 76 Veterans Avenue, Bath, NY 14810, who anticipates not conducting discussions with offerors submitting quotations under the subject solicitation and reserves the right to make contract award based on the initial offer submitted. Requirements may be added by future amendments to this solicitation. The Government contemplates award of a Firm Fixed Price contract resulting from this solicitation. Only offers from companies with a valid GSA schedule contract or GSA teaming agreements for these items will be accepted. Furthermore, IAW FAR 19.5 Set-Asides for Small Business, this solicitation will be set aside to small businesses with valid GSA schedule contracts or valid GSA teaming agreements. SYNOPSIS/SOLICITATION Proposals must be received no later than 1:00 pm, EST on Friday, May 1, 2015. Offers may be submitted via email to Edward Lyke, edward.lyke@va.gov. When submitting offers, please note the solicitation number VA528-15-Q-0175 in subject line of the email. Please note that the offeror assumes all risk for any delay in the transmission of their proposals. Any offers received after the 1:00 PM EST deadline will not be considered. All offerors are encouraged to review FAR 52.212-1, INSTRUCTION TO OFFERORS-COMMERCIAL ITEMS and Internal Instruction, INT L2.11-4, E-MAIL QUOTATIONS and to submit their proposals early. ? This solicitation is being submitted on a "Brand Name or Equal" basis in accordance with the following Federal Acquisition Regulation (FAR) provision. The brand names are annotated in the Statement of Work (SOW). FAR 52.211-6 (a) If an item in this solicitation is identified as "brand name or equal," the purchase description reflects the characteristics and level of quality that will satisfy the Government's needs. The salient physical, functional, or performance characteristics that "equal" products must meet are specified in the solicitation. (b) To be considered for award, offers of "equal" products, including "equal" products of the brand name manufacturer, must-(1) Meet the salient physical, functional, or performance characteristic specified in this solicitation; (2) Clearly identify the item by-(i) Brand name, if any; and (ii) Make or model number; (3) Include descriptive literature such as illustrations, drawings, or a clear reference to previously furnished descriptive data or information available to the Contracting Officer; and (4) Clearly describe any modification the offeror plans to make in a product to make it conform to the solicitation requirements. Mark any descriptive material to clearly show the modification. (c) The Contracting Officer will evaluate "equal" products on the basis of information furnished by the offeror or identified in the offer and reasonably available to the Contracting Officer. The Contracting Officer is not responsible for locating or obtaining any information not identified in the offer. (d) Unless the offeror clearly indicates in its offer that the product being offered is an "equal" product, the offeror shall provide the brand name product referenced in the solicitation.
