- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This is a combined synopsis/solicitation for a Blanket Purchase Agreement (BPA) for commercial items prepared in accordance with the format in FAR Subpart 12. 6, 13.3 and 13.5, Test Program for Certain Commercial Items, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation: quotes are being requested and a written solicitation will not be issued. Solicitation number W81K00-15-T-0186 is issued as a Request for Quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80 and Defense Federal Acquisition Supplement Publication (DPN) 20150226. This acquisition is issued on an UNRESTRICTED basis under NAICS code 339114 and size standard is 500. The Government anticipates awarding a single award issued as Firm Fixed Price BPA. The requirement of this solicitation is to provide Various Dental Implants, Instruments & Supplies at Eisenhower Army Medical Center, Fort Gordon, GA 30905. Delivery starts no later than 30 days after award. All responsible sources may submit a quotation which shall be considered by the agency. BLANKET PURCHASE AGREEMENT BETWEEN Contractor's Name And Eisenhower Army Medical Center, Fort Gordon, GA (EAMC) Contract BPA Number W81K00-15-A-_________ Authority: This agreement is established in accordance with Federal Acquisition Regulation (FAR) Part 13, Subpart 13.303, Blanket Purchase Agreements (BPAs). The BPA will be reviewed by the Contracting Officer at least once annually before the anniversary of the effective date, and revised to conform to all requirements of Statues, Executive Orders, and the FAR. Revisions will be evidenced by a modification to the BPA. (1) Description of Agreement This Blanket Purchase Agreement (BPA) to provide Various Dental Implants, Instruments & Supplies effective for a term of five (5) years starting on the signature date through May 31, 2020 with appropriate updates and reviews. The BPA may be renegotiated at the end of each contract year. Either party may terminate this agreement by giving thirty (30) days written notice before the end of the BPA term. (2) Extent of Obligation (Contractor's name) price list is effective (date of price list) is hereby incorporated into Section J of this BPA as attachments. The price list shall remain effective until a request for revision is submitted, approved and incorporated by a Contracting Officer's duly executed modification. The contractor shall submit BPA price revision requests in writing to the Contracting Officer 30 days, at minimum, prior to the proposed effective date. The government is only obligated to the extent of authorized purchases actually made under this BPA. (3) Purchase Limitations: BPA Call Limit = $1,000.00 BPA Master Dollar Limit = $50,000.00 quote mark Prices of this BPA are set forth in the contractor's published price list which is attached to this BPA, and will be updated annually. Prices shall be based on the contractor's published price list and any discounts offered, and shall be as low, or lower than, those charged to the contractor's most favored customers for comparable quantities of similar items. The Contractor may propose revisions to prices and items offered which shall become effective provided the contracting officer has been given 30 days written notice of and has not objected to the revisions. quote mark (4) Individuals Authorized to Purchase under the BPA a. A list of individuals authorized to purchase under the BPA will be identified on the finalized BPA. b. A call number will be provided by the authorized individual placing the call. (5) Delivery Tickets All packing lists (delivery tickets) shall contain the following information: (i) Name of supplier (ii) BPA Number (iii) Date of Purchase (iv) Purchase Order Number (v) Itemized list of supplies furnished (vi) Quantity, unit price, and extension of each item, less applicable discounts (unit prices and extensions need not be shown when incompatible with the use of automated systems, provided that the invoice is itemized to show this information). (viii) Date of delivery or shipment (6) Invoices/Payment The contractor shall create a separate invoice for each call. A summary invoice shall be submitted, at least monthly, to the card holder or upon expiration of this BPA, whichever occurs, first, for all deliveries made during a billing period, identifying the delivery tickets covered therein, stating their total dollar value, and supported by receipt copies of the delivery tickets. Email or mail invoices to the authorized individual placing the order. Payment will be made by Credit Card. (7) Customer Service (Contractors Point of Contact Information and instructions for placing orders) (8) Notification Concerning Unauthorized Calls Any supplies/services provided pursuant to a call made by unauthorized personnel may result in the non-payment for such products or services, and possible cancellation of the agreement. (9) Promotion Contractor agrees it will not advertise the use of this BPA in such a manner as to state or imply that the Government endorses a product, service, project, or commercial line of endeavor. Quotes are due by 22 May 2015 at 12:00 pm EST. Questions must be submitted via email no later than11 May 2015. Quotes and questions shall be submitted via email to carolyn.f.lovett.civ@mail.mil. All responsible offerors must submit: 1. Price list and any discount terms of dental implants, instruments and supplies identified in the attached spreadsheet. This attached is not inclusive of all supplies that may be required. Therefore please submit your price list for all dental supplies you provide. 2. A completed copy of the representations and certifications at FAR 52.212-3 (see FAR 52.212-3 (b) for those representations and certifications that the offer shall complete electronically) (m) SUBSTITUTE ITEMS: If you plan to provide other than the brand name cited in the purchase description, provide in your response the brand name and the catalog or part number you intend to furnish and the characteristics of the substitute item that are equivalent to the brand name. Also provide all documents which clearly describe the item(s) so that it/they can be evaluated by the end user. Failure to submit the requested substitute item information before the closing date will eliminate the vendor from all further consideration. Items otherwise equal may not satisfy the requirement due to differences in material composition, size, environmental application, etc. If an item is furnished which does not meet the requirement the item will be returned at the vendor's expense for a full refund. Final decision as to award will be based on lowest price technically acceptable (technical acceptability is determined by the end-user).
