Solicitation number W912D1-15-R-0023 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-74 dated 29 May 2014. This is procurement is under NAICS 238130, Wall component (i.e., Exterior, Interior), prefabricated, installation with a size standard $15 million. Camp Arifjan, Kuwait requires the following items. CLIN 0001: Furnish and Install Pre-Fabricated Walls CLIN 0002: DBA Insurance CLIN 0003: Contractor Manpower Reporting (CMR) CLIN 0004: Synchronized Pre-Deployment and Operational Tracker (SPOT) Description of Requirements: Statement of Work (SOW) 1. Camp Arifjan, Kuwait seeks a supplier to Furnish and Install Pre-Fabricated Walls. Vendor(s) shall provide a to scale design drawing of their proposed Wall layout based on the available space and the requirements within that space. Interior Walls: Are to be 118 in' high and 5 in' thick (metal studs covered by standard drywall) with insulation Painted with a flat finish. The doors are to be solid wood core or equivalent. The one office door should be clear or translucent (Glass). 2. Any resulting contract shall be a single award firm fixed price contract. 3. All items shall be FOB destination. 4. There will be a site visit conducted on 1 May 2015 from 1:00-2:00 P.M. (Kuwait Time). All interested parties shall submit names, company and contact information to CPT Stanley R. Burdine at stanley.r.burdine2.mil@mail.mil NLT 17:00 PM (Kuwait Time) on 29 Apr 2015. All interested parties will meet at 12:45 P.M. (Kuwait Time) on 1 May 2015 at Building 209 located on Camp Arifjan, Kuwait. 5. Offeror(s) shall contact CPT LeShawn Torrance in advance to coordinate for the site visit and receive pertinent Camp Arifjan access information. All required documents needed to gain access to Camp Arifjan shall be submitted NLT 2:00 p.m. (Kuwait Time) 28 April 2015. All work performed by the contractor shall be guaranteed for a period of one year from the date of acceptance of defects and workmanship. 9. All persons shall understand English suitable to respond to warnings, emergencies and directions issued by escort personnel. PART 1 GENERAL INFORMATION 1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, tools, materials, supervision, and other items and non-personal services necessary to furnish and install pre-fabricated walls for the 54TH Theater Signal Battalion Regional Network Operations Center (RNOSC) (building 209, room 118) Camp Arifjan, Kuwait. The contractor shall perform to the standards in this contract. 1.2 Background: The RNOSC is currently located in T341. The relocation will provide the RNOSC with a permanent office space. 1.3 Objectives: The contractor shall furnish and install pre-fabricated walls that will be able to provide: a. adequate security measures for securing network operations b. an area for storage of sensitive items and equipment required for RNOSC operations. c. walls will address the security screening of operations, personnel and provide an isolated meeting and storage space. 1.4 Scope: The contractor shall furnish and install pre-fabricated walls in the RNOSC, Camp Arifjan, Kuwait. 1.5Period of Performance: The period of performance shall be for 90 days after contract award. The Period of Performance reads as follows: 90 days after award The Government reserves the right to extend the term of this contract at the prices set forth in Section B in accordance with the terms and conditions contained in clause 52.217-9 entitled, quote mark Option to Extend the Term of the Contact quote mark. 1.6 General Information 1.6.1 Quality Control: The contractor shall develop and maintain an effective quality control program to ensure services are performed in accordance with this SOW. The contractor shall develop and implement procedures to identify, prevent, and ensure non-recurrence of defective services. The contractor's quality control program is the means by which he assures himself that his work complies with the requirement of the contract. As a minimum, the contractor shall develop quality control procedures that address the areas identified in Technical Exhibit 1, quote mark Performance Requirements Summary quote mark. After acceptance of the quality control plan the contractor shall receive the Contracting Officer's Representative acceptance in writing of any proposed change to his QC system. The contractor shall provide a Quality Control Plan to the Contracting Officer Representative (COR) within 10 days after contract award. 1.6.2 Quality Assurance: The Government (COR) shall evaluate the contractor's performance under this contract in accordance with the Quality Assurance Surveillance Plan. This plan is primarily focused on what the Government must do to ensure that the contractor has performed in accordance with the performance standards. It defines how the performance standards will be applied, the frequency of surveillance, and the minimum acceptable defect rate(s). 1.6.3 Government Remedies: The Contracting Officer shall follow FAR 52.212-4, quote mark Contract Terms and Conditions-Commercial Items quote mark or 52.246-4, quote mark Inspection of Services-Fixed Price quote mark for contractor's failure to perform satisfactory services or failure to correct non-conforming services. 1.6.4 Recognized Holidays: The contractor shall provide emergency services on all United States, Kuwait, Qatar Holidays. New Year's DayLabor Day Martin Luther King Jr.'s BirthdayColumbus Day President's DayVeteran's Day Memorial DayThanksgiving Day Independence DayChristmas Day 1.6.5 Hours of Operation/Place of Performance: The contractor duty days/hours will be Saturday thru Thursday 0700-1700 hours in accordance with host nation requirements. Exception is when the Government facility is closed due to local or national emergencies, administrative closings, or similar Government directed facility closings. The Contractor must at all times maintain an adequate work force for the uninterrupted performance of all tasks defined within this Statement of Work when the Government facility is not closed for the above reasons. When hiring personnel, the Contractor shall keep in mind that the stability and continuity of the work force are essential. The contractor's place of performance will be at the Regional Network Operations Center (RNOSC) (building 209, room 118) Camp Arifjan, Kuwait. 1.6.6 Type of Contract: The Government shall award a Fixed Price Contract. 1.6.7 Security Requirements: The contractor shall comply with all U.S. Government, Network Enterprise Technology Command (NETCOM), and Area Support Group Kuwait security requirements for Installation Access. 1.6.7.1 Security: The contractor shall be responsible for safeguarding all Government property provided for contractor use. At the close of each work period, Government facilities, equipment, and materials shall be secured. 1.6.7.1.A Personnel Security/Anti-Terrorism: The Contractor shall become knowledgeable of all current Department of Defense (DOD), State Department, and local command instructions pertaining to personnel security and anti-terrorism issues. Contractor personnel shall complete Level One Anti-Terrorism and OPSEC Level I training within one month after award. On-line training is available from the Government. Contractor personnel shall provide the COR with the Certificate of Training upon completion of the on-line course. AT Level I awareness training and OPSEC Level I training is available at the following website: https://atlevel1.dtic.mil/at and http://cdsetrain.dtic.mil/opsec. quote mark 1.6.7.2 Key Control: The Contractor will not be issued facility keys. 1.6.7.3 Conservation of Utilities: The contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities, which include turning off the water faucets or valves after using the required amount to accomplish cleaning vehicles and equipment. 1.6.8 Special Qualifications: The contractor is responsible for ensuring all employees possess all required engineering, software, and Information Technology licenses. 1.6.9 Safety: The Contractor shall comply with the Army Regulation 385-10, quote mark The Army Safety Program. 1.6.10 Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The Contracting Officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the Contracting Officer will apprise the contractor of how the Government views the contractor's performance and the contractor will apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the Government. 1.6.11 Contracting Officer Representative (COR): The (COR) is identified below: CPT LeShawn Torrance, 54th Theater Signal Battalion, APO 09366, DSN 430-2622 Cell 9721-5246, email: leshawn.f.torrance.mil@mail.mil. The COR monitors all technical aspects of the contract and assists in contract administration The COR is authorized to perform the following functions: assure that the Contractor performs the technical requirements of the contract: perform inspections necessary in connection with contract performance: maintain written and oral communications with the Contractor concerning technical aspects of the contract: issue written interpretations of technical requirements, including Government drawings, designs, specifications: monitor Contractor's performance and notifies both the Contracting Officer and Contractor of any deficiencies; coordinate availability of Government furnished property, and provide site entry of Contractor personnel. A letter of designation issued to the COR, a copy of which is sent to the Contractor, states the responsibilities and limitations of the COR, especially with regard to changes in cost or price, estimates or changes in delivery dates. The COR is not authorized to change any of the terms and conditions of the resulting order. 1.6.12 Contract Manager: The contractor shall provide a contract manager or lead who shall be responsible for the performance. The name of this person and an alternate who shall act for the contractor when the manager is absent shall be designated in writing to the Contracting Officer. The contract manager or alternate shall have full authority to act for the contractor on all contract matters relating to daily operation of this contract. The Contract Manager shall provide a bi-monthly report to the COR detailing previous months preventive maintenance site visits. 1.6.13 Identification of Contractor Employees: All contract personnel attending meetings, answering Government telephones, and working in other situations where their contractor status is not obvious to third parties are required to identify themselves as such to avoid creating an impression in the minds of members of the public that they are Government officials. They must also ensure that all documents or reports produced by contractors are suitably marked as contractor products or that contractor participation is appropriately disclosed. Contractor personnel will obtain Area Support Group-Kuwait (ASG) badges (as applicable) and wear them while on an installation. 1.6.14 The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report ALL contractor manpower (including subcontractor manpower) required for performance of this contract. The contractor is required to completely fill in all the information in the format using the following web address https://cmra.army.pentagon.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical Representative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Estimated direct labor hours (including sub-contractor); (6) Estimated direct labor dollars paid this reporting period (including sub-contractor); (7) Total payments (including subcontractor); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (10) Locations where contractor and sub-contractors perform the work (specified by zip code in the United States and nearest City, Country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contingency contract language, and, (13) Number of contractor and sub-contractor employees deployed in theater this reporting period (by country). As part of its submission, the contractor will also provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period will be the period of performance not to exceed 12 months ending September 30 of each Government fiscal year and must be reported by 31 October of each calendar year. The contractor shall ensure all the employees of the contractor and all sub-contractors are registered in the Synchronized Pre-Deployment and Operational Tracker (SPOT) Program available at https://spot.altess.army.mil or submitted manually to the Contracting Office if directed by the Contracting Officer IAW PWS. SPOT reporting will be updated as required. 1.6.15 Billing Information: 1.6.16 The contractor shall submit an invoice in accordance with the Contract Line Item Number (CLIN) structure established in the contract. The contractor shall utilize the electronic invoicing system, Wide Area Work Flow (WAWF) at https://wawf.ed.mil less than https://wawf.ed.mil/ greater than. The contractor shall have current registration through Central Contractor Registration (CCR), www.ccr.gov less than http://www.ccr.gov/ greater than in order to utilize WAWF. 1.6.17 Sexual Assault and Sexual Harassment Policy: The contractor shall comply with and ensure all employees comply with the Sexual Assault and Sexual Harassment Policy outlined in Attachment1 of this PWS. Sexual Harassment Policy Compliance: Contractor shall certify that all employees that will be performing work in association with this contract have been fully trained per the requirements of Attachment 1. The COR shall be provided with the following information at a minimum: employee's name, civil identification number, and date trained. Proof of Sexual Harassment Policy compliance is due prior to start of work for all employees performing on site. The Contractor shall annually train all employees to prevent sexual assault and sexual harassment. This training must, at a minimum, ensure that all Contractor employees performing work in association with this contract understand the definitions and policy. Each employee's compliance with this training requirement shall be reported to the Contracting Officer Representative prior to employee being allowed access to the worksite. 1.6.18 DBA Proof of Insurance: As noted in the CLINs for this requirement, the Contractor shall provide a policy of Defense Base Act Workers' Compensation to the Contracting Officer. Performance may not start until the notice of a policy from Continental Insurance Co. has been received by the Contracting Officer. The DBA requirement also applies to all subcontractors performing under this SOW. If a subcontractor is performing work under this SOW, the Contractor shall provide evidence of subcontractor's DBA insurance to the Contracting Officer. Upon award, Contractor shall provide a DBA application to Rutherford by emailing application to Mike.dower@rutherfoord.com so that a DBA policy may be issued. PART 2 DEFINITIONS & ACRONYMS PWS - Performance Work Statement SOW- Statement of Work Contracting Officer's Representative (COR): A representative from the requiring activity assigned by the Contracting Officer to perform surveillance and to act as liaison to the contractor CCE- Contracting Center of Excellence MCF- Main Communications Facility Defective Service: A service output that does not meet the standard of performance associated with it in the Performance Work Statement. Quality Assurance Surveillance Plan (QASP): An organized written document specifying the surveillance methodology to be used for surveillance of contractor performance. Quality Control: Those actions taken by a contractor to control the performance of services so that they meet the requirements of the SOW. Quality Assurance: Those actions taken by the Government to assure services meet the requirements of the Performance Work Statement. AOR-Area of Responsibility ASI-Approved Service Interruption Best Effort: That effort expended by the contractor to perform within the awarded ceiling price all work specified in this task order (TO) and all other obligations under this TO and the basic contract. This effort includes providing required qualified personnel, properly supervised, and following industry accepted methodologies and other practices. The effort is further characterized by operating at all times with the Government's best interest in mind, using efficient and effective methods, and demonstrating sound cost control. The effort must be identical to the effort that would be expended if this were a firm-fixed price TO and the contractor's profits were dependent upon reducing costs while meeting the Government's requirements in terms and quality and schedule. Failure to provide this required effort may result in the withholding of payment for hours expended that do not qualify as best effort or a reduction in the rate per hour to reflect decreased value of services received. PART 3 GOVERNMENT FURNISHED PROPERTY, EQUIPMENT, AND SERVICES 3.1GENERAL: The Government will provide the contractor site access and work space. 3.2 Equipment: No 3.3 Services: No 3.1 Utilities: All utilities in the site facility will be available for the contractor's use in performance of duties outlined in this SOW. The Contractor shall instruct employees in utilities conservation practices. The contractor shall be responsible for operating under conditions that preclude the waste of utilities. 3.4 Facilities: No PART 4 CONTRACTOR FURNISHED ITEMS AND SERVICES 4.1 General: Except for those items specifically stated to be Government furnished in Part 3, the contractor shall furnish everything required to perform this PWS. 4.2 Parts and Material: The contractor shall provide all parts and materials. 4.3 Secret Facility Clearance: The Contractor personnel working in the RNOSC (inside buildings) Camp Arifjan, Kuwait shall be escorted at all times. The RNOSC is a secure communications area. At NO time are cell phones or cameras authorized inside buildings. Possession of a cell phone or camera is prohibited and may be grounds for arrest. Personnel not cleared will be escorted by US Government personnel with a clearance at all times. All persons shall understand English suitable to respond to warnings, emergencies and directions issued by escort personnel. 4.4 Warranty: The contractor shall provide a full, unlimited one (1) year warranty for all contractor-provided hardware, software, firmware, materials, provided equipment and workmanship, which begins upon final acceptance by the Government. The contractor life cycle approach shall provide for a full range of services to ensure a comprehensive and responsive logistics support infrastructure to meet the needs and requirements of the Government. Upon contract award, detailed instructions on obtaining warranty, replenishment, Depot, and ETAS support will be provide to the Government. PART 5 SOW SPECIFIC TASKS 5.1 Basic Services: The contractor shall install and furnish pre-fabricated security screening walls. 5.2 The contractor work performance shall be performed according to this SOW. 5.3 The contractor's engineers shall identify themselves to the Contracting Officer Representative (COR), Site OIC or Site Lead upon initial entry to the site. 5.4 General Requirements: Furnish and install pre-fabricated walls. 5.5 Specifications/Work Requirements, Plans, and Approvals: The contractor shall furnish and install pre-fabricated walls in the RNOSC. Final plans will require approval of the COR after contract has been awarded. Plans: and calculations shall be submitted for approval to the COR before the installation begins. Approval of Installations: The completed system shall be inspected, and tested by the COR. Only listed or approved equipment and devices shall be used in the system. 5.7 Furnish and Install: The contractor shall furnish and install pre-fabricated security screening walls in the Regional Network Operations Center (RNOSC) Camp Arifjan, Kuwait. PART 6 APPLICABLE PUBLICATIONS 6.1 Publications applicable to this SOW are listed below. The publications must be coded as mandatory or advisory, the date of the publication, and what chapters or pages are applicable to the requirement. All publications listed are available via the Internet at the specified e-mail address. None TECHNICAL EXHIBIT 1 Performance Requirements Summary The contractor service requirements are summarized into performance objectives that relate directly to mission essential items. The performance threshold briefly describes the minimum acceptable levels of service required for each requirement. These thresholds are critical to mission success.
