Description

Solicitation VA247-15-R-0355 A/E Design Renovate and Upgrade Operating Rooms, Phase 1 Amendment 00005 Date 25 April 2015 The purpose of Amendment A00005 is to address a question asked in response to Solicitation VA247-15-R-0355, A/E Design Renovate and Upgrade Operating Rooms, Phase 1. Question 7: Specialized experience and technical competence - " Submit up to ten (10) relevant projects, accomplished within the past seven (7) years that best demonstrate your relevant experience to the solicitation requirements. Firm is at risk of receiving a lower rating if fewer than four (4) relevant projects are submitted. Also please submit up to four (4) relevant projects, accomplished within the past seven (7) years that best demonstrates the relevant experience for each major support firm (major sub-contractors). Do you want 10 projects - PLUS 4 additional projects? Response 7: The prime firm can submit a maximum of ten (10) relevant projects. A maximum of four (4) relevant projects may be submitted for each support firm (this is optional and not required). The prime firm is at risk of receiving a lower rating if fewer than four (4) relevant projects are submitted for the prime firm. Question 8: Is it the VA's intent to receive Past and Present Performance Questionnaire's from customers of the SDVOSB Prime's projects or from customers of the prime's sub-contractor's projects also which may be represented as sample projects within the SF330? Response 8: The past performance questionnaire is designated for the firm. The composition of the firm can take many forms such as corporation, LLC, Joint Venture, partnering agreement. The prime may also submit, at their discretion, past performance questionnaires from customers of the prime's sub-contractor's projects which may be representative as sample projects within the SF 330. All information submitted will be taken into account appropriately. All other terms remain unchanged
