- Map-Conejos Peak Map 2 Map-Conejos Peak RD-Map 1 Map-divide RD-Map 2 Map-Divide RD Map 1 Service Contract Wage Rates Herbicide Application Report Solicitation Package AG-82AK-S-15-0010 This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation number AG-82AK-S-15-0010 is being issued as a request for proposal. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-73. This project is set aside for small business concerns under NAICS code 115310 Support Activities for Forestry, small business size standard $7.5M. The intent of this contract is to apply herbicides to approximately 146 combined acres of Canada Thistle, Russian Knapweed, hoary Cress(short white top), perennial pepper weed (tall white top, ironweed), cheat grass, Oxeye Daisy, yellow toadflax or other noxious weeds or invasive weed species listed by the State of Colorado. Project sites are located on the Rio Grande National Forest in South central Colorado. Item 1 - Conejos Peak Ranger District-noxious weed treatment, 58 acres Item 2 - Divide Ranger District-noxious weed treatment, 88 acres Period of Performance: Approximately May 26 - September 15, 2015. The following provisions apply to this acquisition: Provision 52.212-1, Instructions to Offerors-Commercial Provision 52.212-2, Evaluation-Commercial Items Provision 52.212-3, Offeror Representations and Certifications-Commercial Items The following clauses apply to this acquisition: 52.212-4, Contract Terms and Conditions-Commercial Items 52.212-5, Contract Terms and Conditions Required To Implement Statutes or Executive Orders-Commercial Items. Additional applicable clauses can be found in Section I of the solicitation package. AGAR 452.209 - 71 Assurance Regarding Felony Conviction or Tax Delinquent Status for Corporate Applicants-Alternate 1 (FEB 2012) and AGAR 452.209 - 70 Representation by Corporations Regarding an Unpaid Delinquent Tax Liability or a Felony Conviction (FEB 2012) apply to this acquisition. SAM registration must be completed to receive contract award. Offerors must be registered in System for Award Management (SAM) https://www.sam.gov Offerors must respond to all criteria requested. Offerors who fail to meet these requirements will not be considered for award. B.
