- Wage Determination 2005-2407 Responsibility Questionnaire Appendix A Sports Officials RFQ Appendix B - Bylaws Sports Officials PWS Description: (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested, and a written solicitation will not be issued. (ii) The solicitation number F3C3MD5082AW01 is being issued as a request for quotation. (iii) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-81. (iv) This acquisition is 100 percent set aside for small businesses. The North American Industry Classification System (NAICS) code is 711219 with a size standard not to exceed $11.0 million (U.S.) (v) Sports Officials for basketball, over 30 basketball, volleyball, softball, outdoor soccer, indoor soccer, and flag football. Sports Officials will have a base year with an additional four option years. See attached Request for Quotations. (vi) See Attached Performance Work Statement (PWS) for the description of requirements. (vii) Place of performance is Minot AFB, ND 58705 and period of performance will be 1 August 2015 through 31 July 2016, with the possibility of 4 option years. (viii) The provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. (ix) The provision 52.212-2, Evaluation-Commercial Items, applies to this procurement. The government will award a firm-fixed price contract resulting from this solicitation to the responsible Offeror whose offer conforming to the solicitation will be most advantageous to the government, price and other factors considered. Award shall be based on best value to the government. The evaluation procedure to be used for this procurement is price and price related factors which will be evaluated on a pass/fail basis. The price related factors are as follows: a. Factor 1 - This factor is met if the Offeror has successfully completed two contracts with a minimum value of $10,000 annually and has been actively engaged as a contractor performing services as defined by the NAICS Code 711219 within the last five years. Complete attached Responsibility Questionnaire. b. Factor 2 - This factor is met when the offeror provides a detailed method of recruiting, hiring, and retaining qualified personnel who will be able to perform the service identified in the Performance Work Statment. (x) Offerors are also reminded to complete Offeror Representations and Certifications on-line at https://www.sam.gov/portal/public/sam as required in provision 52.212-3 prior to submitting an offer. (xi) Clause 52.212-4, Contract Terms and Conditions Commercial Items, applies to this acquisition. (xii) Clause 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items, applies to this acquisition and the additional FAR clauses cited in the clause are applicable: 52.204-10, 52.219-6, 52.219-8, 52.219-28, 52.222-3, 21, 26, 35, 36, 37, 50, 52.232-18, 33, 52.222-41, and 42.
