Description

REQUEST FOR QUOTATION: Contractor shall provide all tools, equipment, labor, supervision, transportation, and materials to complete the annual preventive maintenance, inspection, and testing in accordance with the manufacture's recommendation and accepted industry standards on all ASCO proprietary Switchgear for the C.W. Bill Young Health Care System in accordance with the (IAW) the attached Statement of Work (SOW). The Period of Performance is for a twelve (12) month base year with four (4) 12 month option periods. Preventative maintenance shall be accomplished within 45 calendar days from date of award. Contractor shall be an authorized ASCO service provider and also have technical training, track records of working experience in the preventative maintenance, inspection and testing of ASCO Switchgear and its components in healthcare, industrial, educational and commercial facilities for a minimum of five (5) continuous years. Method of payment will be Electronic Payment by Electronic Funds Transfer. Upon completion of work all Invoices from the contractor shall be submitted electronically in accordance with VAAR Clause 852.232-72 Electronic Submission of Payment Requests. Discounted pricing is requested. Quotes must represent best value and result in the lowest overall cost alternative (including technical acceptable) in accordance with the requirements and delivery. The agency will award a Firm-Fixed Price (FFP) contract using the LOWEST PRICE TECHNICALLY ACCEPTABLE (LPTA) methodology. The Government reserves the right to make award without discussions. To receive an award, contractor must be registered in the System for Award Management (SAM) database (www.sam.gov). QUESTIONS: All questions regarding this RFQ must be in writing and may be sent by e?mail to steven.scharlow@va.gov. Questions must be received no later than 5/4/2015, 8:00 AM EST. No further questions will be accepted after that date and time. Answers to vendor questions will be posted on FBO no later than 5/4/2015 at 5:30 PM EST. QUOTES: Quotes must be received on/or before the 5/7/2015, 11:00 AM EST to be considered for award. Quotes shall be submitted by electronic mail to: steven.scharlow@va.gov.
