Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation number 0010644278 is hereby issued as a Request for Quote (RFQ). The incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-81, Effective 10 April 2015. This acquisition is 100% set aside for Small Business competition concerns and is under North American Industry Classification Standards (NAICS) code 333415 with the number of employees not to exceed 750. Quotes received from concerns that are not Small Business concerns shall be considered nonresponsive and will be rejected (in accordance with FAR 52.219-6 Notice of Total Small Business Set-Aside which applies to this solicitation). Description of Requirement: The contractor shall move 2 existing walk in refrigerators and provide and install two (2) new walk in refrigerators within the Bassett Army Community Hospital (BACH), Medical Supply Warehouse, Fort Wainwright, Alaska 99703. Quote must include all shipping and handling fees and charges to move the two existing refrigerators and install the two new refrigerators. This will include disconnecting, reconnecting, and testing all electrical, temperature sensors, and alarm systems associated with the walk-in refrigerators. A complete list of objectives is included in Section 1.3 and Part 4 of the Performance Work Statement (PWS) in Attachment 1-PWS. All work is to be done at On-Site location on Fort Wainwright, AK 99703. Questions: Any questions must be received no later than May 12, 2015, 4:00 PM Alaska Time. BASIS FOR AWARD: A Firm-Fixed Price award will be issued in writing. Telephone bids will not be processed. Award shall be made only to contractors who have registered with System for Award Management (SAM). Vendors may register at: http://www.sam.gov. Contract will be awarded on the Lowest Price Technically Acceptable offer that meets the requirements listed in this solicitation and the PWS. CERTIFICATIONS REQUIRED FOR AWARD: 1. All Vendors responding to this RFQ must provide an authorization letter by the manufacturer of the brand of refrigerators that you will be providing stating that your company is an authorized installer /repairer of that brand of refrigerator. The purpose of this is so that the warranty will not be voided by having the refrigerator installed by an unauthorized installer. 2. All responding Vendors must also submit an Air Conditioner Repairer (or equivalent) Certification showing that they are authorized to use refrigerant gases.
