- A15PS00454 Amendment No.: 0002 is for Solicitation No.: A15PS00454 is as follows: 1. To INCLUDE THE SYNOPSIS: THIS IS A BUY INDIAN SET ASIDE The Bureau of Indian Affairs (BIA) is soliciting offers to procure for construction supplies for Division of Transportation, Force Accounts Farmington, NM. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. Solicitation No. A14PS00454, herein cited is issued as a Request for Quote (RFQ) and will be procured under FAR Part 12, Acquisition of Commercial Items. Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80. This acquisition is for a Native American Owned Business Small Business Set-Aside under the Buy Indian Act (25 U.S.C). The NAICS Code is 212321, Size standards 500. Scope of Work: Contractor shall provide all labor, materials, supervision and incidentals necessary to deliver Hot Mix Asphalt meeting the requirements of Transportation Standard Specifications for road and Bridge Construction most recent edition. Section 406, Asphaltic Concrete, 3#4# Inch (750 tons), Base Mix (200 tons) and Section 407, Asphaltic concrete friction course (200 tons), AND: To provide Aggregate Base meeting the Arizona Department of Transportation Specifications for Road and Bridge construction most recent edition Section 303, Aggregate Bases Aggregate Base shall be Class 2 per Table 303-1 (300 tons). The sequence of construction is: Once all Aggregate Base is completed in place, Base Mix will be placed in it's entirely followed by ¾" mix and finally Asphaltic concrete Friction Course. Aggregate Base shall be delivered one Belly Dump Trucks at times and locations as designated by the Construction Manager. Hot Mix Asphalt shall be delivered in end dump trucks at times and locations as designated by the Construction Manager. This project is located on US 160 near Mile Post 418 in Dennehotso, Arizona. Period of Performance is from date of award through September 30, 2015. Applicable Taxes $ __________ Total: $_________________ Vendor to deliver the followiing hot mix asphalt products in end dump trucks to a project located in Dennehotso, Arizona All materials must meet ADOT specifications, mix design and pertinent data need to be submitted for Acceptance prior to the delivery of material Delivery quantities and dates will vary depending on work in progress so all deliveries must be coordinated with the Construction Manager. Award will be made at a fair and reasonable price to the responsive and responsible offeror whose quote conforming to the requirements herein will be the most advantageous to the Government, price and other factors considered. CONTRACT CLAUSES: The following clauses apply to this acquisition: FAR 52.212-1, Instructions to Offerors Commercial Items; FAR 52.212-3, Offeror Representations and Certifications Commercial Items; FAR 52.212-4, Contract Terms and Conditions Commercial Items; and FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders Commercial Items.
