- Statement of Objectives (SOO) The Air Force Medical Operation Agency (AFMOA) and the Air Force Installation Contracting Agency (AFICA), 773 rd Enterprise Sourcing Squadron (773rd ESS/PKB) intends to procure non-personal commercial services in support of the AFMOA Readiness Support Branch (SGALX). The SGALX in support of the Air Force Medical Logistics Division (SGAL) and the Air Force Medical Service (AFMS) requires services for the War Reserve Materiel (WRM) program to provide calibration, preventive maintenance, and repair services (on an as needed basis) for AFMS medical test, measurement, and diagnostic equipment (TMDE) inventory. This is a solicitation for commercial services in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice and Attachment 01, Statement of Objectives (SOO). Proposals will be requested after posting of the Request for Proposal (RFP). The applicable NAICS Code is 811219, with a Size Standard of $20.5 Million (FAR 19.502-2(a)). This will be a new requirement and the (RFP) is issued under FA8052-15-R-0007 as Full and Open Competition. The evaluation methodology will be Technically Acceptable with Performance and Price Tradeoff (PPT) source selection in accordance with FAR Part 15.101-1. It is the Government's intents to award a single contract to the Offeror who overall provides the best value offer to the Air Force as determined by an integrated assessment of the evaluation criteria establish in section L & M of this RFP, and in the addendum to provision FAR 52.212-2 - Evaluation of Commercial Items. Technical acceptability will be evaluated and given ratings of acceptable or unacceptable. Unacceptable offers will not be eligible for award. Past Performance will be evaluated as significantly more important than Price. Solicitation Questions: Questions relating to the synopsis/solicitation and /or terms and conditions shall be submitted in writing by email or fax only to individuals identified herein NLT 1:00 PM, Local San Antonio time on Monday, 25 May 2015. Any questions received after this time/date may not be answered due to time constraints of the solicitation. Proposals are due NLT 3:00 PM, local San Antonio TX time on Wednesday, 03 June 2015. Estimated award date is 19 June 2015 with performance to begin 01 July 2015. The resulting award will be Firm-Fixed (FFP) contract, with a twelve (12) month base period and four (4) - one year options, if all options are exercised. The Period of Performance (PoP) is scheduled to begin 01 July 20125, The RFP is schedule to be release on about 04 May 2015. ADDITIONAL INFORMATION AND GOVERNMENT RIGHTS 1. Communications: Communications conducted to resolve minor or clerical errors will not constitute discussions and the Contracting Officer reserves the right to award a contract without the opportunity for proposal revision. Offerors are cautioned to submit sufficient information and in the format specified on Addendum to FAR Provision 52.212-1, Instructions, Conditions, and Notices to Offerors as stated in Section L of this RFP. 2. The Offeror's proposal shall include all data and information requested and must be submitted in accordance with these instructions. Failure to submit proposals as outlined herein may render a vendor ineligible for award. The government reserves the right to make award based on proposals received with or without discussions. 3. The Government intends to award without conducting discussions. Offerors should not assume they will be contacted or afforded an opportunity to qualify, discuss or revise their proposals. Therefore, each initial proposal should contain the Offeror's best terms from a technical standpoint. However, the Government reserves the right to open and conduct discussions if necessary at any time at the discretion of the Contracting Officer. Discussions would then be held with all Offerors within the competitive range all offers had been evaluated. 4. Contractual Document: A contract issued by the Contracting Officer, signed by the Contracting Officer, is the only instrument by which Government funds are obligated. The Government reserves the right to cancel this solicitation, either before or after the closing date. If you proceed with work prior to receipt of a fully executed contract, you do so at our own risk. 5. Period of Acceptance of Offers. The Offeror agrees to hold the prices in its offer firm for 90 calendar days from the date specified for receipts of offers. 6. Responses to this posting received by the 773 rd ESS/PK after the submittal deadline and time indicated will not be considered. The Offeror assumes the risk of the method of dispatch chosen. Postmarking by the submittal date and time shall not substitute for actual response receipt. 7. Also include in the proposal that a Wide Area Work Flow (WAWF) account has been initiated or already established. The Offeror must be registered and current in the System for Award Management (SAM), available at www.sam.gov, with no exemptions. All invoices shall be submitted using Wide Area Work Flow (WAWF). •2. 9. Under no circumstances shall the 773 rd ESS/PK be responsible for any preparation expenses, submission costs, or any other expenses costs, or damages, of whatever nature incurred as a result of the respondent's participation in this posting process. Respondent understands and agrees that it submits its proposal at its own risk and expense, and releases 773 rd ESS/PK from any claim for damages or other liability arising out of the posting process. The 773 rd /PK shall not be liable for any errors in the Offeror's response. The Offeror is responsible for careful review of its entire response to ensure that all information or informational material submitted in response to this posting becomes the property of the 77rd ESS/PK and shall not be returned.
