Description
Local Competition Memo Solicitation Package Date Issued: April 30, 2015 Question Deadline: May 11, 2015 Closing Date: June 15, 2015 SUBJECT: Request for Proposal (RFP) SOL-663-15-000001 Water Installation for Five Health Centers TO ALL PROSPECTIVE OFFERORS: The United States Agency for International Development (USAID) in Ethiopia conjunction with the Offices of Acquisition and Assistance (OAA) and Health, AIDS, Population and Nutrition (HAPN) office invites interested and qualified local offerors to submit proposals in accordance with the requirements of this RFP. The selected offeror will be required to install and connect water supply at five Health Centers (Udassa, Wacho, Sibiya, Jaradado, and Mehal Jarso) in the SNNP Region. The details of this project are defined in the scope of work (SOW) located in Section C of the attached Request for Proposal (RFP). This procurement will be conducted pursuant to Part 15 of the Federal Acquisition Regulation (FAR) (48 CFR Chapter 1). Offers must be submitted in accordance with the instruction listed in Section L of this RFP. USAID will select the offeror's proposal that demonstrates the technical understanding required to achieve the objectives at the most reasonable cost to the United States Government. USAID/Ethiopia anticipates awarding a firm fixed price (FFP) contract as a result of this solicitation. The period of performance for this award will be for a 42 weeks (subject to availability of funds and successful performance). The Government is not obligated to make an award as a result of issuance of this solicitation. Offerors are requested to submit proposals that are both realistic and reasonable for proposed work. The total estimated cost for performing this activity is between $100,000 - $135,000. Offerors are directed to pay particularly close attention to instructions listed in both Section L - Instructions to Offerors and Section M - Evaluation Factor for Award. All submitted proposals must be signed by an authorized official with the authority to bind the Offeror's organization. Please also pay careful attention to, and complete Section K - Representations, Certifications and Acknowledgements. It is extremely important that all hardcopy and electronic submissions be made by the designated date and time indicated in Block 13 of the RFP cover page (Standard Form 1442). Proposals received after the closing date and time will be processed as late and handled in accordance with FAR 15.208. Electronic submissions must be sent to the following address: caddis@usaid.gov no later than 4:00 PM local (Addis Ababa) time on Monday June 15, 2015. In the subject field of the email, you must put: SOL-663-15-000001 Water Installation for Five Health Centers. No other form of submission will be accepted. The USAID email system can only accept up to 8 megabytes. If your submission is larger, you must send multiple emails. They must be additionally labeled in the subject field with the transmission number and then the total number being transmitted, for example for a transmission that has three emails and attachments: 1/3, 2/3, 3/3, etc. USAID would also like to invite all interested applicants to a discussion session to be held on May 7, 2015, from 10:00am-12:00noon at the US Embassy in Addis Ababa, Ethiopia. All interested participants should notify the Mission and send names of all attendees to caddis@usaid.gov by Monday May 4, 2015, so that access to the Embassy compound is requested. In addition, any questions regarding this RFP are to be submitted in writing to caddis@usaid.gov, by the date and time specified herein (May 11, 2015). Answers to questions received in writing will be made available to all offerors through their email addresses. Issuance of this solicitation does not obligate the U.S. Government to make an award, nor does it commit the U.S. Government to pay for costs incurred in the preparation and submission of a proposal.
