- Sole Make and Model Justification PLEASE READ THIS ENTIRE NOTICE CAREFULLY AS IT CONSTITUTES THE ONLY NOTICE THAT WILL BE ISSUED. This is a combined synopsis and solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The Request for Quotation (RFQ) number is N66604-15-Q-1707. The resultant contract will be a firm-fixed price contract. The Naval Undersea Warfare Center Division, Newport (NUWCDIVNPT) intends to purchase the following Avaya Platinum Partners Support service of sole make and model*; no substitutes/third party service providers allowed (refer to the attached Sole Make and Model Justification): _______________________________________________ Tier 1 and Tier 2 remote support - Total Quantity of 235 Hours •· Support is all remote •· All support provided will take place Monday-Friday, between 08:00 - 17:00 EST •· Government customer will call into the Technical Support line •· Four (4) hour response time required for minor situations •· Three (3) hour response time required for major alarms •· All materials including cabling, wall-field and patch cords to be provided by the Government •· Assume start date of 15 May 2015 *OFFERORS MUST BE AN AUTHORIZED AVAYA PLATINUM PARTNER. THE GOVERNMENT WILL VERIFY THAT THE APPARENT AWARDEE HAS A VALID AND CURRENT RESELLER AGREEMENT WITH AVAYA THAT AUTHORIZES THE PLATINUM PARTNER TO SELL TO THE FEDERAL GOVERNMENT PRIOR TO AWARD. OFFERORS WHO ARE NOT AVAYA PLATINUM PARTNERS WILL NOT MEET THE MINIMUM ELIGIBILITY CRITERIA AND WILL BE CONSIDERED NON-RESPONSIVE. The Government will make payment in arrears. This procurement is solicited as a 100% Small Business set aside. The North American Industry Classification System (NAICS) Code for this acquisition is 811231. The Small Business Size Standard is $11M. Incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-81. The provision at FAR 52.212-1, Instructions to Offerors - Commercial Items applies to this solicitation. The provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items applies to this solicitation. Clauses 52.212-4, Contract Terms and Conditions - Commercial Items, 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items apply to this solicitation. This requirement will be awarded to the offeror with the lowest price technically acceptable (LPTA) quote. Technically acceptable means the service quoted matches the sole make and model items listed above (no substitutions allowed) in the quantity specified. Additional terms and conditions: Defense Priorities and Allocations System (DPAS) rating is DO-C9; Clause C26 "Information Assurance - Unclassified DoD Information on Non-DoD Information Systems" applies (refer to attached clause); this requirement has an exception to Electronic and Information Technology (EIT) Accessibility Standards ; Clause C57 applies (see attached clause). ATTACHMENT 1 ------------------------------------------------------------------------------------------------ C26 INFORMATION ASSURANCE - UNCLASSIFIED DOD INFORMATION ON NON-DOD INFORMATION SYSTEMS (JUL 2012) (a) The Contractor shall ensure that unclassified DoD information it receives or produces in support of DoD activities is protected according to the information safeguards described in Attachment 2 to Directive-Type Memorandum (DTM) 08.027 - Security of Unclassified DoD Information on Non-DoD Information Systems, which is available at the following web address: http://www.dtic.mil/whs/directives/corres/pdf/858201p.pdf (b) Upon request by the Government, the Contractor shall provide documentation demonstrating the safeguards the contractor has implemented to ensure the security of unclassified DoD Information. (c) Contractor personnel that have not been briefed on Attachment 2 to Directive-Type memorandum (DTM) 08-027 - Security of Unclassified DoD Information on Non-DoD Information Systems shall be denied access to contractor systems that contain unclassified DoD information. (d) Subcontracts. If the Contractor issues any subcontracts in which the subcontractor will have access to unclassified DoD information, the Contractor shall include this clause. ---------------------------------------------------------------------------------------- C57 EXCEPTION FROM ELECTRONIC AND INFORMATION TECHNOLOGY ACCESSIBILITY REQUIREMENTS (JUN 2001) (a) The Government has determined that this procurement is an exception to the Electronic and Information Technology (EIT) Accessibility Standards (36 C.F.R. § 1194).
