- This is a competitive combined synopsis/solicitation for commercial items prepared in accordance with Federal Acquisition Regulation (FAR) Subpart 12.6. This announcement will include a Standard Form, SF1449 Solicitation Document. The Solicitation Number for this Acquisition is VA244-15-Q-0782, and is being issued with this announcement as a Request for Quote (RFQ). The Solicitation is being conducted under the procedures of FAR Part 12 and FAR Part 13. The NAICS Code for this Acquisition is 339112. The Small Business Size Standard is 500 Employees. The Government intends to award a new Firm Fixed-Price Contract, for the one time furnishing of the Verathon Medical Titanium Glidescope System Items, including Blades, that are listed in the attached RFQ, in May 2015. All responsible sources may submit a capability statement, written proposal, or quotation which shall be considered by the Agency. Closing Response Date is 05/06/2015 at 4:00PM Eastern Time. The Agency Point of Contact is Ronald Mazzei, at ronald.mazzei@va.gov. All respondents must be registered in the System for Award Management (SAM) prior to award of the Contract. All respondents must fill in the Cost-Price Schedule, in the SF1449 Form Solicitation, with their quoted prices for all Line Items, and insert their total price for all Line Items at the end of the Cost/Price Schedule where provided. Offerors shall include a completed copy of the provision at FAR 52.212-3, Offeror Representation and Certifications - Commercial Items, with its offer, or indicate in the body of the transmittal email that they are registered in SAM, and what their Cage Code in SAM is. FAR Clause 52.212-4, Contract Terms and Conditions - Commercial Items, and FAR Clause 52.212-5 - Contract Terms and Conditions Required to Implement Statues and Executive Orders - Commercial Items, applies to this acquisition. Any Contractor Terms and Conditions of Sale must be accepted by the Contracting Officer, and can be made part of the Contract, but if they are, they will only apply to the extent they do not conflict with the Terms and Conditions of the Contract, or any Federal Laws, Rules, and Regulations.
