Description
Statement of Work Background: Think Delirium 2.0 is a clinical demonstration project (CDP) funded by VISN 8, and required as part of the FY2015 MOU between the Gainesville VA GRECC and the VISN 8 Network Office. It is a clinical training initiative targeting multidisciplinary VA clinical staff, aimed at promoting awareness, identification, and treatment of Delirium in the older Veteran population. The Virtual Dementia Tour (VDT) is a simulation learning experience that enables participants to experience some of the sensory limitations and confusion often associated with cognitive decline and diseases of aging. The VDT is one component of the Think Delirium CDP, which fulfills the project's objectives. Objective: The NF/SG VHS GRECC is seeking to train VA staff as facilitators of the Virtual Dementia Tour. These staff will participate as facilitators of the tour at scheduled education/training activities in FY15, required per the 2015 MOU between the VISN 8 Network Office and the Gainesville GRECC. SCOPE OF WORK A. A Virtual Dementia Tour (VDT) Certified Trainer will conduct the VDT Facilitator training for up to 15 participants. B. The vendor will provide North Florida/South Georgia Veterans Heath System GRECC with all training materials necessary for a comprehensive training experience. C. The vendor will provide 5.5 NAB CEUs for VDT Facilitator Training participants. D. The vendor will provide North Florida/South Georgia Veterans Heath System GRECC with a comprehensive list of items needed for the facilitation of the Tour experience. This list will be provided to North Florida South/Georgia Veterans Health System GRECC at least one month in advance of the training date. E. The VDT Certified Trainer will provide all VDT components necessary to conduct the Virtual Dementia Tour. F. A training venue of sufficient size to accommodate the number of anticipated attendees will be provided by the North Florida/South Georgia Veterans Heath System GRECC. G. North Florida/South Georgia Veterans Heath System GRECC will provide A/V equipment. H. North Florida/South Georgia Veterans Heath System GRECC will provide up to 4 volunteers to assist with the VDT experience during the morning prior to the Facilitator Training. Qualifications/Certifications "A Certified Trainer and support personnel in the Virtual Dementia Tour are required. Safety Regulations "All training of VA staff to become VDT facilitators will be performed in the Gainesville VA Simulation Lab, which is fully equipped with the safe and necessary infrastructure required to perform a training of this nature. Scheduling and Hours of Work "All work in areas involved shall be scheduled with the COR. "Work shall be scheduled for proper execution to completion of this contract. "Hours of work. The contractor will perform work during regular VA tour of duty hours. Evaluation Factors The evaluation factors for this service contractor are as follows in order of priority. Technical Capability, Past Performance, and Cost.
