Description

Urology Stone Baskets Source of Sought Information The Department of Veterans Affairs Strategic Acquisition Center (SAC) is issuing this sources sought announcement as a means of conducting market research to identify parties having an interest in and the resources to support the requirement for Urology Stone Baskets. The result of this market research will contribute to determining the method of procurement. The applicable North American Industry Classification System (NAICS) code assigned to this procurement is 339112 for Surgical and Medical Instrument Manufacturing. The Product Service Code is 6515 Medical and Surgical Instruments, Equipment and Supplies. This notice is for planning purposes and a solicitation is not available at this time. This request for capability information does not constitute a request for proposal, quote, or bid. Submission of any information in response to this market survey is voluntary and the Government assumes no financial responsibility for any cost incurred. This announcement is to gain knowledge of potential qualified sources relative to NACIS 339112. Responses will be used by the Government to make appropriate acquisition decisions. Federal Supply Schedule holders, Service-Disabled Veteran Owned Small Business, Veteran Owned Small Business, HUB-Zone Small Business, Small Disadvantaged Business, and Women-Owned Small Business, are encouraged to respond. PRODUCT IDENTIFICATION: Each response must be able to comply with the Minimum Technical Requirement (MTR) for Urology Stone Baskets listed below. Capability Responses must include: 1.Company Name. 2.Socio-economic status and business size. Specifically, indicate whether your business is a certified Service Disabled Veteran Owned Small Business (SDVOSB) and registered in VetBiz. Registry at http://vip.vetbiz.gov, or a Veteran Owned Small Business (VOSB). 3.Small Businesses; indicate if your firm qualifies as a (a) small, emerging business, or small disadvantage business, (b) If disadvantaged, specify under which disadvantaged group and if your firm is certified under Section 8(a) of the Small Business Act. 4.If small are you a distributor or a manufacturer. 5.Manufacturers please provide your authorized distributors. 6.Provide the DUNS number of your firm. 7.State whether your firm is registered with the System for Award Management (SAM) at http://www.sam.gov. 8.Specify if your company have a Federal Supply Schedule on GSA and if so provide the link to the VA FSS information, including contract number, point of contact, and commodity schedule listing the specified items above. 9.Are all items referenced on the MTR Table available on Schedule? 10.Provide any current, (within the past 2 years), past performance information. Include contract number, point of contact name, telephone number, or email address. 11.Provide product information, literature, or electronic catalog information. 12.Sub-contracting intentions, include set asides. 13.Do you participate with the Department of Veteran Affair Prime Vendor Program? 14.Is the product made in America? Responses are due by 2:00 PM EDT, 14 May 2015. Please include in the subject line of your email the following: "Attention: Urology Stone Baskets". This notice is to assist the VA in market research identifying potential sources only. Issuance of this notice does not constitute an obligation on the part of the Government to procure these commodities or to issue a solicitation nor to notify respondents of the results of this notice. No solicitation document exists at this time. The VA is neither seeking proposals nor accepting unsolicited proposals, and responses to this notice cannot be accepted as offers. Any information the vendor considers proprietary should be clearly marked as such. The Government will not pay for any information or administrative costs incurred in response to this Sources Sought Notice.
