- LIMITED SOURCES JUSTIFICATION ORDER >$150,000 FAR PART 8.405-6 2237 Transaction # or Vista Equipment Transaction #: 436-15-2-112-0306, 575-15-2-035-0188,554-15-3-023-0492, and 436-15-2-820-0084 This acquisition is conducted under the authority of the Multiple Award Schedule Program. The material or service listed in par. 3 below is sole source, therefore, consideration of the number of contractors required by FAR Subpart 8.4 - Federal Supply Schedules, is precluded for the reasons indicated below. Restricted to the following source: Polymedco 510 FURNACE DOCK RD CORTLANDT MANOR, NEW YORK 10566 1 The requested material or service represents the minimum requirements of the Government. (1) AGENCY AND CONTRACTING ACTIVITY: Department of Veterans Affairs, VISN 19 Network Contracting Office 19 4100 E. Mississippi Ave, STE 900 Glendale, CO 80246 (2) NATURE AND/OR DESCRIPTION OF ACTION BEING APPROVED: This is a sole source firm fixed price BPA request for Polymedco to provide Fort Harrison VAMC, Billings VA Clinic, Cheyenne VAMC, and Grand Junction VAMC with Polymedco OC-AUTO Micro 80 FIT testing analyzer or the or OC-Sensor Diana automated system. The period of performance is for a base year and four option years starting from 1 May 2015 and ending 30 April 2020. (3)(a)A DESCRIPTION OF THE SUPPLIES OR SERVICES REQUIRED TO MEET THE AGENCY'S NEED: The requirement is for Polymedco to continue to provide OC-AUTO Micro 80 FIT testing analyzers and the OC-Sensor Diana automated systems. Cost Per Reportable Results (CPRR) are conducted on a test basis where OC FIT-CHEK is used for detecting gastrointestinal bleeding associated with disorders such as colorectal cancer, polyps and colitis. (b)ESTIMATED DOLLAR VALUE: $364,268.50 (c)REQUIRED DELIVERY DATE: 1 May 20 15 and ending 30 April 2020 (4)IDENTIFICATION OF THE JUSTIFICATION RATIONALE (SEE FAR 8.405-6), AND IF APPLICABLE, A DEMONSTRATION OF THE PROPOSED CONTRACTOR'S UNIQUE QUALIFICATIONS TO PROVIDE THE REQUIRED SUPPLY OR SERVICE. 1Specific characteristics of the material or service that limit the availability to a sole source (unique features, function of the item, etc.). Describe in detail why only this suggested source can furnish the requirements to the exclusion of other sources. Polymedco is the only vendor that offers automated analyzer for this testing. This is the main capability that meets the VISN medical station's requirement. Other vendors/manufacturers require the technologist to interpret and record the reaction manually which increases the chance for error. The following functions are fully automated with Polymedco capability: Method detects only HUMAN HEMOGLOBIN so dietary restrictions are not required for collection of samples. o 80 samples per hour o Runs up to 10 samples per rack o Results available in eight minutes o Easy calibration and automatic serial dilution o Start mode o 999 sample memory o 3" L x 1" wide LCD display o Closed sampling bottle o Eliminates Exposure to Bio-hazardous material o Optimizes Test Sensitivity and Specificity o Easy implementation in lab o Half day installation o Never come in contact with the sample o Eliminates subjectivity of read o Patient results are automatically updated into their records, eliminating transcription errors o Compact design (57 lbs., 12.5" x 21" x 16.5") is ideal for a small workstation o New immunoassay CPT code o National Medicare limit: $23.22 o Samples can be bar coded o Ability to interface to an LIS (RS232 Port and Written to HL7 (5) DESCRIBE WHY YOU BELIEVE THE ORDER REPRESENTS THE BEST VALUE CONSISTENT WITH FAR 8.4 TO AID THE CONTRACTING OFFICER IN MAKING THIS BEST VALUE DETERMINATION: Polymedco provides services as a cost per reportable result through a GSA Federal Supply Contract that has already been determined fair and reasonable in accordance with FAR Part 8.4. They provide a full service automation capability that no other vendor can provide at competitive prices; therefore, they offer the best value to the VA in meeting the needs of the Government. (6) DESCRIBE THE MARKET RESEARCH CONDUCTED AMONG SCHEDULE HOLDERS AND THE RESULTS OR A STATEMENT OF THE REASON MARKET RESEARCH WAS NOT CONDUCTED: The vendors below provide similar testing method; however, they require manual testing and do not provide automation which increases the potential for error and increased manual labor. The Government will continue to monitor all FSS schedules available for new entrants providing microbiology products, services, and technologies.
