Description
This is a Sources Sought Announcement seeking approved Food and Drug Administration (FDA) suppliers of Cochlear Implants and associated speech processors, accessories, parts, warranty, and commercial procedures for handling these types of medical devices. This notice is expected to be followed by a formal solicitation announcement, number to be determined, on or about March 15, 2016. The Department of Veterans Affairs (VA) Denver Acquisition & Logistic Center (DALC), in support of the Veterans Health Administration (VHA) and the Department of Defense (DoD), has requirements for Cochlear Implants and associated items that are expected to be satisfied with Indefinite Quantity/Indefinite Delivery contracts awarded to two or more qualified manufacturers. The contracts are anticipated to cover national requirements for a base year and four one-year option periods. Manufacturers must be approved to supply Cochlear Implants by the FDA. All items must be commercially available. The DALC wants to determine if Service Disabled Veteran Owned Small Businesses, Veteran Owned Small Businesses, and Small Businesses are interested and are FDA-approved manufacturers of Cochlear Implants. In order to help make this determination, the VA DALC needs to know the following: (a) company name (b) address (c) point of contact (d) phone, fax, and email (e) DUNS number (f) Cage Code (g) Tax ID Number (h) Type of business, e.g. Services Disabled Veteran Owned Small Business, Veteran-Owned Small Business, or Small Business. A copy of your FDA approval to supply Cochlear Implants must be included with your response. Interested parties are encouraged to submit this information electronically to email address michael.sanford2@va.gov or by mail to Dept. of Veterans Affairs, Denver Acquisition & Logistics (003A4D1), Attn: Michael Sanford, PO Box 25166, Denver, CO 80225. Questions may be directed to Michael Sanford at (303) 273-6212. Responses are due by 4:00 pm. Eastern Time on May 22, 2015. THIS IS NOT A REQUEST FOR PROPOSAL, QUOTATION, OR BID, AND DOES NOT OBLIGATE THE GOVERNMENT TO AWARD A CONTRACT. REQUESTS FOR A SOLICIATION WILL NOT RECEIVE A RESPONSE. THE VA DALC IS NOT SEEKING PRICING OR OFFERS OF ANY KIND. THE VA RESERVES THE RIGHT TO SOLICIT THESE ITEMS FROM LARGE OR SMALL BUSINESSES, AS APPROPRIATE.
