Description

THIS SOURCES SOUGHT NOTICE IS NOT A REQUEST FOR PROPOSAL (RFP). NO SOLICITATION IS BEING ISSUED AT THIS TIME. THIS NOTICE SHALL NOT BE CONSTRUED AS A COMMITMENT BY THE GOVERNMENT TO ISSUE A SOLICITATION OR ULTIMATELY AWARD A CONTRACT, NOR DOES IT RESTRICT THE GOVERNMENT TO A PARTICULAR ACQUISITION APPROACH. ANY INFORMATION PROVIDED TO THE GOVERNMENT AS A RESULT OF THIS SOURCES SOUGHT NOTICE IS VOLUNTARY. RESPONSES WILL NOT BE RETURNED. NO ENTITLEMENT TO PAYMENT OF DIRECT OR INDIRECT COSTS OR CHARGES BY THE GOVERNMENT WILL ARISE AS A RESULT OF CONTRACTOR SUBMISSION OF RESPONSES OR THE GOVERNMENT'S USE OF SUCH INFORMATION. This Sources Sought Notice is for planning purposes only. Contractor shall furnish all labor, materials, parts, equipment, tools, transportation, and supervision to provide, install at the Michael E DeBakey VA Medical Center, 2002 Holcombe Blvd, Houston, Texas 77030. The Contractor shall provide all necessary training to the designated VA employees on how to use the installed equipment/products after the installation and setup were complete. All equipment shall be brand new, and all work shall be done in a first-class manner. The contract price shall be firm-fixed, all inclusive not limited to specified products/equipment, installation, setup/configuration, and training on how to use the installed products/equipment. Contractor shall furnish all labor, materials, supervision, to deliver minimum or maximum supplies to the Michael E. DeBakey VAMC. The L15600 - X-Porte Ultrasound Kiosk System with Stand and Options, and L25X / 13-6 MHZ Transducer Transverse Biopsy Compatible, and HFL50x / 15-6 MHZ Transducer Biopsy Compatible are the only device determined to be able to meet the specifications established for the proper patient care and similar Sonsite unit have been in use at the MEDVAMC. INSTRUCTIONS FOR SUBMISSION After review of the above information, interested parties may submit a response in an electronic format only via email bruce.lundvall@va.gov not later than May 11, 2015 12:00 pm CST. Electronic files should be in Microsoft Word or PDF format. ALL INFORMATION SHOULD BE UNCLASSIFIED MATERIAL ONLY. Respondents should include as part of their submission: a.A synopsis of the company's capabilities to provide qualified contracting support personnel on company letterhead. b.Company point of contact telephone number and email address. c.Socioeconomic status Any company proprietary information must be marked as such. The total number of pages for the response is limited to 10 one-sided 8 ½ x 11 pages with one inch margins and font no smaller than 12 point.
