- RFQ SOW SCA This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a separate written solicitation will not be issued. This combined synopsis/solicitation is issued as a Request for Quote (RFQ) for F4D4A15037A001. Submit written quotes in accordance with (IAW) CLIN structure in announcement - Oral quotes will not be accepted. All firms or individuals responding must be registered in System for Award Management (SAM) (https://www.sam.gov). This is a 100% small business set aside. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-81 and DFARS DPN 20150420. North American Industrial Classification Standard (NAICS) 562112 and Size Standard of $38.5M apply to this procurement. Project Number: F4D4A15037A001 Project Title: MEDICAL WASTE DISPOSAL 1. Please see attached Statement of Work (SOW) dated 16 April 2015 for detailed description of required duties. 2. Quotes shall be valid until 13 October 2015. 3. Period of Performance will be from 1 October 2015 through 30 September 2016, Base Year, with four (4) one-year options from 1 October 2016 - 30 September 2020 plus one six-month extension through 31 March 2021. 4. All questions or comments relating to this solicitation must be provided to the Contract Specialist (CS) in writing via email NLT 12:00 noon PST on 21 May 2015. 5. Quotes must be received no later than 12:00 pm PST on 03 June 2015 via attached Request for Quote (RFQ). 6. Quotes shall conform to the CLIN structure as established in this synopsis/solicitation.
