THIS IS A SMALL & LARGE BUSINESS SOURCES SOUGHT NOTICE REPRESENTING A MARKET SURVEY AND IS NOT A REQUEST FOR PROPOSALS, PROPOSAL ABSTRACTS, QUOTATIONS OR INVITATION FOR BIDS The Department of Veterans Affairs VISN 7 Network Contracting Activity is conducting a MARKET SURVEY to obtain information regarding: (1) the availability and capability of qualified small or large business sources; (2) whether they are Service-Disabled Veteran-Owned, Veteran-Owned, Small Business, HUB Zone 8(a), Women-Owned, or small disadvantaged business concerns; and (3) their size classification relative to the North American Industry Classification System (NAICS) code for the proposed acquisition. Your responses to the information requested will assist the Government in determining the appropriate acquisition method, including whether a set-aside is possible. An organization that is not considered a small business under the applicable NAICS code should submit a response to this notice. Business concerns should be capable of furnishing all labor and materials/parts as requested to provide a Dark Adaptometer to measure the dark adaptation in early-stage retinal disease by the Research Lab located at the Atlanta Veterans Affairs Medical Center in Atlanta, Georgia. The NAICS code is 339112 DESCRIPTION OF THE REQUIREMENT: The ADAPTDx is needed by several investigators and research laboratories in the VA RR&D Center for Visual & Neurocognitive Rehabilitation because it will enhance and streamline procedures compared to other time intensive measurements of dark adaptation which is advantageous to our investigators looking to incorporate the measure into their study protocols. We have several current research proposals that require quick means to utilize dark adaptometry to characterize the effect of exercise intervention on the pre-post dark adaptation functions of participants. It will be used to sensitively measure dark adaptation in early-stage retinal disease. This is because dark adaptation is strongly impacted by retinal insult long before visual acuity loss reaches measurable levels. It measures the recovery time sensitivity to night vision that serves as a vital sign in assessing early Age-related Macular Degeneration. POTENTIAL SOURCES SHALL PROVIDE THE FOLLOWING INFORMATION IN THEIR RESPONSE: 1) Company name, address, phone number, primary contact(s), e-mail address, NAICS code(s), business size (i.e., small/large) and DUNS Number. 2) Provide a Statement of Capability that demonstrates the offeror's past performance in providing Building Automation System (BAS) Metasys. Please include the following: (a) staff expertise including their availability, experience, and formal and other training; (b) current in-house capability and capacity to perform the work; (c) prior completed projects of similar nature; (d) corporate experience and management capability; and (e) examples of prior completed Government contracts, references, and other related information. 3) Submit Capability Statements by Tuesday, May 5, 2015 to: Department of Veterans Affairs, VISN 7 Network Contracting Activity ATTN: Demetrius D. Kittrell, Atlanta, GA 30030. Responses should be received no later than 2:00 PM EST. Submissions may be made via email to: demetrius.kittrell@va.gov NO CAPABILITY STATEMENTS MAY BE SUBMITTED AFTER AUGUST 11, 2010. AT THIS TIME NO SOLICITATION EXISTS. (DO NOT REQUEST A COPY OF THE SOLICITATION).
