- This amendment is executed to correct a quantity error on the combined synopsis/solicitation posted on Fedbizopps.gov. This is a combined synopsis/solicitation for commercial items prepared in accordance with format Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation HSFLGL-15-Q-00201 is issued as a Request for Quote (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 05-80. This procurement is a 100% Small Business set-aside and the associated NAICS code 339920 with Size Standard 500 employees. The Federal Law Enforcement Training Centers (FLETC) has a requirement for the following supplies: Delivery: 30 Days Delivery ARO, FOB Destination Address: 2400 Glynco Pkwy, Shipping & Receiving Warehouse, Glynco, GA 31524 Requirement: BRAND NAME OR EQUAL CLIN 0001 - Quantity: 200, Unit: Each, Pro Force Helmet with face cage, size medium, black in color, Manufacturer: Pro Force, Item Number: 8927 in accordance with the attached minimum requirements CLIN 0002 - Quantity: 200, Unit: Each, Pro Force Helmet with face cage, size Large, black in color, Manufacturer: Pro Force, Item Number: 8928 in accordance with the attached minimum requirements CLIN 0003 - Quantity: 110, Unit: Each, Pro Force helmet with face cage, size XLarge, Manufacturer: Pro Force, Item Number: 8929 in accordance with the attached minimum requirements CLIN 0004 - Quantity: 1, Unit: Each, SHIPPING FOB 30 days ARO Submission of Information: 1) Provide product specifications and photo attachments with pricing quote. Failure to provide the requested documents will be determined non-responsive offer. Product sample may be requested for further evaluation. 2) Name of Company, Address and DUNS Number. 3) Point of Contact, email and Phone Number. The selected Offeror must comply with the following commercial item terms and conditions, which are incorporated herein by reference: FAR 52.211-6 Brand name or Equal, FAR 52.212-1, Instructions to Offerors - Commercial Items, applies to this acquisition; FAR Part 13.106 evaluation procedures apply.
