Description
The Department of Veterans Affairs Medical Center, Network Contracting Office (NCO) 6, on behalf of Hampton VA Medical Center, is conducting market research to determine small business, small disadvantaged business, 8(a), Service Disabled Veteran Owned Small Business (SDVOSB), Woman Owned Small Business or HUBZone's interest for transitional male housing within fifteen miles of the Hampton VA Medical Center. The Department of Veterans Affairs, Hampton VA Medical Center located at 100 Emancipation Drive, Hampton, Virginia 23667 is seeking transitional housing services for male Veterans who are chronically homeless and carry a diagnosis of a serious mental illness or substance use disorder. The purpose of this housing is to help chronically homeless Veterans to adjust to living in a non-institutional, home-like setting, in preparation for independent housing. The facility must be located within fifteen miles of the Hampton VA Medical Center and shall have up to seven available beds. The Contractor shall furnish room and board, meals, laundry facilities, access to transportation, and personal hygiene items. Therapeutic and Rehabilitative Services shall be provided to include activities to include structured group activities, including physical activities as appropriate, instruction in, and assistance with, health and personal hygiene, monitoring of medications to include monitoring of locked drawers for the storage of medications, medication administration records, and weekly medication checks. Supportive social services, and intensive case management, in collaboration with the Hampton VA Medical Center Case Managers, shall include: Intake/Assessment, Individual Care Plan, Discharge Planning to include housing search assistance, Care Coordination, Resource/Referral, Assistance with Social Security and other benefit claims processes (SOAR), assistance with money management (develop a budget), community reintegration, and linkage to non-VA resources for recreation, spiritual and social support. The facility must have a current occupancy permit or license required by the authority having jurisdiction and warrant that all applicable fire laws are being complied with and there are no recommendations of fire officials, which have not been resolved. The North American Industry Classification System (NAICS) Code for this acquisition is 623220, Size Standard $15.0 M, and the Classification code is G009. THIS IS A SOURCES SOUGHT NOTICE ONLY. This notice does NOT constitute a request for proposal, request for quote, or invitation for bid. No formal solicitation document exists at this time. The Government will not pay for any information solicited.
