- Editable Sections for Solicitation Number DOL-OPS-15-R-00019 SECOP Seattle - Solicitation Number DOL-OPS-15-R-00019 The Department of Labor (DOL) Office of the Assistant Secretary for Administration and Management (OASAM) Office of Procurement Services (OPS) hereby issues Solicitation Number DOL-OPS-15-R-00019 for the procurement of Medical Second Opinion and Medical Examination Services (SECOP) for the Seattle District Office (District Office 14) to support the DOL Office of Workers' Compensation Programs (OWCP) Division of Federal Employees' Compensation (DFEC). The Government anticipates the award of a single, Firm Fixed Price, Indefinite Delivery / Indefinite Quantity (IDIQ) Contract. This requirement is posted under the North American Industry Classification System (NAICS) Code of 621111 -- Offices of Physicians (except Mental Health Specialists). The Small Business Size Standard for NAICS Code 621111 is $11 Million. The Product Service Code (PSC) for the requirement is Q999. The requirement for Medical Second Opinion and Medical Examination Services (SECOP) for District Office 14 -- Seattle, shall be competed utilizing Full and Open Competition. PROPOSAL SUBMISSION: Please refer to Solicitation Number DOL-OPS-15-R-00019 for all instructions, conditions, notices, and general guidance related to this procurement. The majority of Solicitation Number DOL-OPS-15-R-00019 is also provided in an "editable" format to allow Offerors to use any relevant attachments or text for their proposal response. Please refer to the uploaded document titled: "Solicitation Sections - Solicitation Number DOL-OPS-15-R-00019". Offerors are expected to thoroughly review and comply with the attached solicitation. Failure to do so will be at the Offeror's own risk. Please note the following due dates related to Solicitation Number DOL-OPS-15-R-00019: 1. THE DUE DATE FOR QUESTIONS AND COMMENTS CONCERNING THIS RFP is MONDAY, MAY 18, 2015, 5:00 P.M. EASTERN TIME. 2. ALL PROPOSAL VOLUMES MUST BE RECEIVED BY THE GOVERNMENT BY 5:00 P.M. EASTERN TIME ON FRIDAY, JUNE 12, 2015. 3. The following documents are attached in support of this Request for Proposal (RFP): A. Solicitation Number DOL-OPS-15-R-00019 B. Solicitation Sections - Solicitation Number DOL-OPS-15-R-00019 It is the ultimate responsibility of the Offeror to ensure delivery of their proposal and all related documents to the designated point of contact via e-mail on or before the closing date and time. The solicitation may be downloaded from the FedBizOpps website listed above. No paper copy / hard copy of the solicitation will be distributed. Prospective Offerors MUST be actively registered in the System for Award Management (SAM) to be eligible for award. The SAM website can be found at: www.sam.gov. The Government anticipates that the period of performance for the contract will consist of one (1) twelve (12) month Base Year and four (4) twelve (12) month Option Years. The place of performance shall be the area served by OWCP's Seattle District Office (Alaska, Idaho, Oregon and Washington). The DOL will use competitive acquisition procedures utilizing best practices for conducting a competitive procurement as authorized by the Federal Acquisition Regulations (FAR). Offers will be evaluated and the contract will be awarded based on the proposal representing the "Best Value to the Government". The evaluation Factors and their relative importance are stated in Solicitation Number DOL-OPS-15-R-00019. Issuance of the solicitation does not constitute an award commitment on the part of the Government, nor does it commit the Government to pay for costs incurred in the preparation and submission of proposals.
