G15PS00396 Attachment 2 - SCA Wage Determination Attachment 1 - Additional Clauses and Provisions COMBINED SYNOPSIS/SOLICITATION (I) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. (II) This solicitation is issued as a request for quotation (RFQ). Submit written quotes on RFQ Number G15PS00396. This solicitation will utilize the policies contained in the Federal Acquisition Regulation (FAR) Part 12 in conjunction with the policies and procedures for solicitation evaluation and award prescribed in FAR Part 13.5, Simplified Acquisition Procedures, as appropriate for this acquisition. (III) The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-81. (IV) The solicitation is not a small business set-aside. The associated NAICS code is 238220. The small business size standard is $15.0 million. (V) This combined solicitation/synopsis is for purchase of the following commercial item(s): 1. Base Year: CLIN NO. 00010 - Services, non-personal, to provide on-site emergency repair and maintenance services on an as needed basis for the National Ice Core Laboratory (NICL) freezer building and refrigeration systems in accordance with the attached Statement of Work. Payment shall be made monthly in arrears. Invoices shall be submitted by the contractor monthly in arrears. Labor shall be identified as separate line items on invoices. Note to offeror's: The estimated hours are as follows: 1. Regular Time: Hours are estimated NTE 62 hours X hourly rate of = Total amount. 2. Over Time: Hours are estimated NTE 10 hours X hourly rate of = Total amount. 3. Holiday Time: Hours are estimated NTE 3 hours X hourly rate of = Total amount. 2. Base Year: CLIN NO. 00020 - Parts Payment shall be made monthly in arrears. Invoices shall be submitted by the contractor monthly in arrears. Repair or replacement parts, and billable supplies shall be identified as separate line items on invoices. Parts and supplies shall be identified and their acquisition be approved by the on-site COR. Parts are estimated not to exceed, therefore the Government will obligate funding for parts at the time of award. 3. Base Year: CLIN 00030 - Services, non-personal, to provide monthly Preventative Maintenance Services in accordance with the attached Statement of Work. Offerors shall include a firm-fixed price per month X 12 months = Total amount. 4. Base Year: CLIN 00040 - Trip Charge (If Applicable) Payment shall be made monthly in arrears. Note to offeror's: Estimated number of trips NTE 8 trips X trip charge = Total amount. 5. Option Year I: CLIN NO. 00050 - Services, non-personal, to provide on-site emergency repair and maintenance services on an as needed basis for the National Ice Core Laboratory (NICL) freezer building and refrigeration systems in accordance with the attached Statement of Work. Option Year II: CLIN NO. 00090 ¿ Services, non-personal, to provide on-site emergency repair and maintenance services on an as needed basis for the National Ice Core Laboratory (NICL) freezer building and refrigeration systems in accordance with the attached Statement of Work. Payment shall be made monthly in arrears. Invoices shall be submitted by the contractor monthly in arrears. Labor shall be identified as separate line items on invoices. Option Year II: CLIN 00110 - Services, non-personal, to provide monthly Preventative Maintenance Services in accordance with the attached Statement of Work. Offerors shall include a firm-fixed price per month X 12 months = Total amount. 12. Option Year II: CLIN 00120 - Trip Charge (If Applicable) Payment shall be made monthly in arrears. Note to offeror's: Estimated number of trips NTE 8 trips X trip charge = Total amount. (VI) Refer to Attachment 1 ¿ Statement of Work for the line item description. (VII) Period of Performance: The period of performance for the Base Year shall commence on June 1, 2015 through May 31, 2016, with each subsequent Option Year consisting of one year. Place of performance is U.S. Geological Survey, National Ice Core Laboratory, W 6th Ave Kipling St., Denver Federal Center, Bldg. 810, MS 975, Denver, CO 80225-0046. (VIII) FAR 52.212-1- Instructions to Offerors - Commercial Items (APR 2014), applies to this acquisition. (IX) FAR 52.212-2, Evaluation - Commercial Items (OCT 2014), applies to this acquisition. (a) The Government will conduct a technical evaluation on the submitted quotations using the non-price factor listed below, assigning a rating of Pass or Fail. Each quote must be determined acceptable under each factor in order to be considered for award. Quotations will be evaluated for acceptability but not ranked using the non-cost/price factors. The following factors shall be used to evaluate offers: Factor A: Technical Capabilities of Proposed Technical Personnel: Offerors shall provide qualifications of proposed technical personnel with experience in performing repair and maintenance services of this type of freezer and refrigeration system and components covered by the statement of work contained herein. Offerors quotation shall include information which demonstrates that the offeror has the necessary understanding, experience, management, manpower, technician certification and education to successfully accomplish the proposed work covered by the statement of work contained herein. In addition, Offerors shall submit documentation required by the statement of work under Section H. - Contractor Qualifications. (b) Options. The government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option/s. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. (X) The offeror shall submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certifications - Commercial Items (MAR 2015), with its quote. An offeror shall complete only paragraph (b) of this provision if the offeror has completed the annual representations and certifications electronically at https://www.sam.gov If an offeror has not completed the annual representations and certifications electronically at SAM, the offeror shall complete only paragraphs (c) through (m) of this provision.
