Description
Well Abandonment and Sealing Document IH Lab Report Guideline for Contractors Drawings Safety Procedures Asbestos Report Specifications Lead Analysis Statement of Work (i) This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) This solicitation, SB1341-14-RQ-0092 is issued as a request for proposal (RFP). (iii) This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. (iv) This solicitation is 100% Small Business Set Aside. NAICS Code: 238910 with a size standard of 14 Million Dollars. (v) This combined synopsis/solicitation is for the following commercial services: 0001: Contractor shall provide all labor materials and equipment to perform wrecking construction per attached documents: 1) Statement of work 2) Drawings 3) Specifications 4) Asbestos and lead paint reports. The contractor shall remove and dispose superstructure of Building 419 to Maryland State approved recycling and/or disposal facility. 0002: Contractor shall provide a health and safety plan within 14 days of contract award. (vi) See attached documents titled: Statement of Work, Specifications, Drawings, Asbestos Report, Lead Report, Lead Analysis, Lab Report. (vii) Services shall be completed within 30 days of award per statement of work. (viii) The provision at FAR 52.212-1, Instructions to Offerors - Commercial, applies to this acquisition. (ix) FAR 52.212-2, Evaluation -- Commercial Items, does not apply to this acquisition. Please see Evaluation Criteria below. (x) The offeror must have completed the Online Representations and Certifications Application (ORCA) which is the provision at FAR 52.212-3, Offeror Representations and Certifications -- Commercial Items. Reps and certs must be uploaded to the System for Award Management at www.sam.gov. (xi) FAR 52.212-4, Contract Terms and Conditions -- Commercial Items, applies to this acquisition. (xii) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items applies to this acquisition. (xiii) The following clauses are also applicable to this acquisition: CAR 1352.209-74, CAR 1352.209-73, CAR 1352-201-70, FAR 52.217-5, NIST 04 Local Billing Clause. (xv) Key Personnel may not be substituted without prior approval of Contracting Officer. (xvi) Quotes are required to be received no later than 5:00 PM Eastern Time, Tuesday, June 9, 2015 and must include the following: Company name, address, phone numbers, DUNS number, itemized quote. Quotes shall be delivered via email to Karen.Armstrong@nist.gov with the title, "Proposal for SB1341-14-RQ-0092" on all submissions. (xvii) A one-time site visit will be held on May 19, 2015 at 2:00pm. If interested in attending, please contact Karen Armstrong at Karen.Armstrong@NIST.gov by Friday, May 15th at 2:00pm. All attendees will be required to submit full name and citizenship via email prior to being granted access to the NIST facility. (xviii) Please direct any questions regarding this solicitation to Karen Armstrong at Karen.Armstrong@nist.gov. Only questions received via email, titled "RFI for SB1341-14-RQ-0092" submitted on or before May 22, 2015 at 3:00pm will be accepted. NO PHONE CALLS WILL BE ACCEPTED FOR THIS REQUIREMENT. Addendum to 52.212-1 Proposal Preparation The proposals shall be submitted in TWO SEPARATE volumes, Volume I-Technical and Volume II-Price. Volume I-Technical: (i) The technical proposal consists of the offeror's proposal delineating its capabilities and how it intends to perform contract requirements. The Technical proposal will be evaluated in accordance with the criteria contained in the evaluation factors for award. (ii) The offeror shall submit the following in order to be considered for possible award: FACTOR A - BONDING The offeror shall provide a Bonding Capacity Letter from the surety or bonding agent showing the maximum bonding amount. FACTOR B - EMR Any offeror must have a CURRENT insurance experience modification rating (EMR) of 1.0 or less. The offeror must submit proof of its current EMR from the National Council on Compensation Insurance (NCCI) or its calculating State Agency for each team member. If not eligible for NCCI rating, then the offeror must explain why and present other proof from their insurance company. Proof from an insurance company will ONLY be used if ineligible for NCCI rating. FACTOR C - EXPERIENCE The offeror shall provide the following, at a minimum, for Factor B Experience. Project descriptions shall include the following information at a minimum: a) Project title, location, and full description addressing all of the charcteristics identified in the Evaluation Factors for Award; b) Project coordination responsibilities and number of sub-contracts held. c) Contract value (at award and completion;) d) Contract value of any Owner Requested Change Orders (at completion;) e) Contract duration (at award and final completion;) f) Amount and extent of self-performed work (% or dollar amount;) g) Amount and extent of subcontracting, by major trades; h) Name of firm's Project Field Supervisor for project; FACTOR D - KEY PERSONNEL Provide information for the proposed Project Field Supervisor that documents that he or she has at least four (4) years of experience as a field supervisor or foreman and has worked on at least one of the projects submitted under the Corporate. The information may be submitted as a resume and/or a narrative form. FACTOR E - PAST PERFORMANCE Offeror shall provide description of projects performed as a prime contractor for at least three (3) similar projects with a contract value greater than $300,000 each in the past four (4) years or currently underway which involve space renovation in an occupied building. Volume II- Price: FACTOR F - PRICE The offeror shall submit a price for each CLIN (CLINs 0001 and 0002) and a total price for the requirement. No other pricing information shall be submitted. The offer shall also submit any other information required to complete the contract. Each offeror shall also complete and submit the following documentation, which must be submitted by the date and time specified for receipt of offers: Annual Representations and Certifications: Offerors should complete Section K-Representations, Certifications, and the Submission of Other Information via System for Award Management (SAM) website: https://www.sam.gov/portal/public/SAM/. EVALUATION FACTORS FOR AWARD This requirement is being procured using FAR Part 13, Simplified Acquisition Procedures. Basis of award will be Lowest Price Technically Acceptable (LPTA). Award will be made to the offeror deemed responsible in accordance with FAR 9.104, whose proposal conforms to the solicitation's requirements and is rated technically acceptable in all evaluation factors, and is the lowest price, trade-offs are not permitted. A finding of unacceptable in any single evaluation factor renders the entire proposal unacceptable and therefore not available for award. The following factors shall be used to evaluate offers: FACTOR A - BONDING FACTOR B - EMR FACTOR C - EXPERIENCE FACTOR D - KEY PERSONNEL FACTOR E - PAST PERFORMANCE FACTOR F - PRICE FACTOR A - BONDING - PASS/FAIL The Government will evaluate the proposal to determine that the offeror has demonstrated that they can obtain a bond in an amount equal to that of their proposal; FACTOR B - EMR - PASS/FAIL The Government will affirm that the Offeror provided proof of its CURRENT insurance Experience Modification Rating (EMR) of 1.0 or less from the National Council on Compensation Insurance (NCCI) or its calculating State Agency. If ineligible for NCCI rating, then the offeror must explain why and present other proof from their insurance company. FACTOR C -EXPERIENCE - PASS/FAIL The Government will evaluate Corporate Experience. The proposal shall be evaluated to determine that the offeror has demonstrated experience, in the past three years, performing at least a single contract: (1) With a minimum contract value of $25,000; AND (2) Completed a commercial building demolition or residential structure razing. Approximate square footage of facility should be at least 5,000 square feet AND (3) Foundation removal digging, excavation, land clearing, land leveling, line slashing or cutting (except maintenance ), concrete breaking and cutting for demolition was included AND (4) Experience with utility removal of: • Electrical service back to the utility point of connection, and • natural gas service back to the utility point of connection, and • service back to the utility point of connection, and • potable water back to system mains. FACTOR D - KEY PERSONNEL The offeror has demonstrated that the proposed Project Field Supervisor has a minimum of four (4) years of experience as a field supervisor or foreman and has worked on at least one of the projects submitted under the Experience evaluation factor FACTOR E - PAST PERFORMANCE - PASS/FAIL Each offeror will be evaluated on its past performance. Information utilized may be obtained from the references listed in the proposal identified for Evaluation Factor C- Experience, other customers known to the Government, CPARS (if available), and others who may have useful and relevant information. The Government will focus on past performance on similar procurements. More recent work may be considered more relevant and more important. Evaluation of past performance will be based on consideration of all relevant facts and circumstances. The evaluation will be based on demonstrated past performance in quality of product or service, schedule and business relationships. The offeror is authorized to provide information on problems encountered on the identified contracts and the offeror corrective actions taken. In the case of an offeror that does not have past contract performance information, or with respect to which information on past contract performance is not available, the offeror shall receive a neutral rating on the factor of past performance. Offeror with no relevant past performance shall not be rated favorably or unfavorably. In order to be considered technically acceptable, an offeror must receive a "Pass" rating in all technical factors. FACTOR F-PRICE The proposed price will be evaluated. The price evaluation will determine whether the proposed prices are realistic, complete, and reasonable in relation to the solicitation requirements and that the offeror is compliant with the Limitation on Subcontracting Clause (Reference Section I.) The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option. Proposed prices must be entirely compatible with the technical proposal.
