Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-72. The North American Industry Classification System Code is 339113, Surgical Appliance and Supplies Manufacturing. The small business size standard is 500 Employees. This procurement is a total set-aside for small business concerns. SPECIFIC CONTRACT REQUIREMENTS: Contractor shall provide the supplies/equipment, per the attached solicitation VA251-15-Q-0331, at the Richard L. Roudebush VA Medical Center, 1481 W. 10th Street, Indianapolis, IN, 46202. The provision at 52.212-1, Instructions to Offerors - Commercial (APR 2014), applies to this acquisition. The provision at 52.212-2, Evaluation - Commercial Items (OCT 2014), applies to this procurement. Quotes shall be evaluated on price, technical and past performance. Price is significantly more important. Offerors are required to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, (MAR 2015), with its offer.
