- Attachment 5 - Cover Sheet Attachment 4 - Local Clauses Attachment 3 - Offer Schedule Attachment 1 - Statement of Work This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number F2FF115055A001 is hereby issued as a Request for Proposal. This combined synopsis/solicitation and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-81, effective 10 Apr 2015. This acquisition is being conducted using Simplified Acquisition Procedures under the authority of FAR 13.5. This acquisition is 100% set-aside for Small Business Concerns with a North American Industry Classification System (NAICS) code of 611519 and size standard of $15M. The Standard Industrial Classification for this requirement is 8299. Subject requirement is to provide all management, tools, equipment, supplies, and labor necessary to ensure the services are performed at Contractors facility. The contractor shall accomplish all National Registry of Emergency Medical Technician - Paramedic Training Course tasks to meet the requirements of this Statement of Work. Additionally the contractor shall provide training and certification for Air Force Special Operations Command (AFSOC) medical personnel. Contractor will provide training for up to 7 students per year. Courses will be accessible throughout the year to allow for flexible scheduling. The accelerated paramedic course length will not exceed 120 calendar days. Offerors shall provide pricing as follows: Contract Line Item (CLIN) 0001, National Registry of Emergency Technician-Paramedic IAW the attached SOW. Quantity 7 Each Base Year: POP Contract Award - 30 June 2016 Contract Line Item (CLIN) 1001, National Registry of Emergency Technician-Paramedic IAW the attached SOW. Offerors shall not submit pricing for CLIN 5001, they will be filled in by the Government for evaluation purposes only and will not be included in the total awarded value of the contract. (d) Comptroller General Examination of Record The Contractor shall comply with the provisions of this paragraph (d) if this contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records -- Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to and right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of type and regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (e) (1) Notwithstanding the requirements of the clauses in paragraphs (a), (b), (c) and (d) of this clause, the Contractor is not required to flow down any FAR clause, other than those in this paragraph (e)(1) in a subcontract for commercial items. Failure to meet a requirement may result in an offer being ineligible for award. Offerors must clearly identify any exception to the solicitation terms and conditions and provide complete accompanying rationale. Technical Requirements in accordance with paragraph 3.0 of the SOW - Technical qualifications will be rated acceptable if the offeror's submittal meets all requirements stated within this section. 1.1 Offeror shall demonstrate their ability to provide three classes per period of performance and meet a Course length not exceed 120 days. 1.2 Offeror shall provide a listing of all course instructors intended for use on the subject contract and provide a copy of their certification in the National Registry of Emergency Technician-Paramedic (NREMT-P) level. 1.3 Offeror shall demonstrate how the course meets or exceeds (NREMT-P) level requirements. 1.4 Offeror shall certify their understanding and concurrence to provide, at no cost, 1 (one) NREMT-P testing opportunity after graduation of the course. 1.5 Offeror shall certify their understanding and concurrence to provide, at no cost, remedial training for first time failure of the NREMT-P exam. 1.6 Offeror shall certify their understanding and concurrence to not have read ahead modules or computer based training as a prerequisite. 1.7 Offeror must demonstrate how the Course curriculum provides certification in Advanced Cardiac Life Support (ACLS), Pre-hospital Trauma Life Support (PHTLS) and Pediatric Education for Prehospital Professionals (PEPP). 1.8 Offeror shall identify the course materials to be provided. 1.9 Offeror shall identify the types of special uniforms and personal protective equipment for training. 1.10 Offeror shall demonstrate their understanding and ability to prorate fees if students are unable to complete the course due to USAF mission requirements or personal hardship. 1.11 Offeror shall provide a Course Content outline meeting the following guidelines: 1.11.1.1 National Emergency Medical Services Education Standards - Paramedic Instructional Guidelines (387 pages). http://www.ems.gov/pdf/811077e.pdf 1.11.1.2 National Emergency Medical Services Education Standards - Advanced Emergency Medical Technician Instructional Guidelines (151 pages). http://www.ems.gov/pdf/811077d.pdf b. Additional Technical Evaluation Criteria: 1.1 Offeror shall demonstrate an 85% or better first test pass rate of the NREMT-P testing Offerors shall not simply restate Statement of Work (SOW) verbiage, but explain how they intend to fulfill the SOW requirements. 1. Pages exceeding the page limitations set forth in this section will not be read or evaluated, and will be removed from the proposal. 2. The technical proposal shall be on 8 ½" x 11" paper except for foldouts used for charts, tables, or diagrams, which may not exceed 11" x 17". A page is defined as one face of a sheet of paper containing information and excludes such things as blank pages, title pages, tables of contents, tabs, dividers, and indices. All pages shall have a minimum margin of 1" on the sides and 1/2" on top and bottom. (excluding document numbers, classification markings, and page numbers). All material must be contained within the page limits identified for each volume. Type shall be 12-pitch font. Text lines shall be spaced a minimum of 1 1/2 spaces. Elaborate formats, bindings or color presentations are not desired or required. 4. PART IV- Wage Determination Submission (As Necessary) 52.222-49 -- Service Contract Labor Standards -- Place of Performance Unknown. The following factors shall be used to evaluate offers: (1) PRICE (2) TECHNICAL Technical is approximately equal to price (b) Options. The government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The government may determine that an offer is unacceptable if the option price(s) are significantly unbalanced. Evaluation of options shall not obligate the government to exercise the option(s) For evaluation purposes of the pricing for CLINS 52.217-8, see paragraph E below. (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. ADDENDUM TO 52.212-2, "EVALUATION-COMMERCIAL ITEMS (OCT 2014) A. This acquisition will utilize a LPTA approach, in which the Government intends to award to an offeror with a technically acceptable l proposal and the lowest overall total evaluated price. The LPTA approach does not permit tradeoffs between price/cost and past performance, and will include an evaluation for technical acceptability. B. Proposals that are unrealistic in terms of technical acceptability or price may be rejected at any time during the evaluation process. The Government reserves the right to make an award without discussions; however, discussions may be conducted with all, some, or none of the offerors. Offerors may be required to participate in telephone discussions or in face-to-face oral discussions at the 1st Special Operations Contracting Squadron, 350 Tully Street, Hurlburt Field, FL. C. The Government intends to award a Firm-Fixed-Price contract as a result of this solicitation. D. Offerors are cautioned to follow the detailed instructions fully and carefully, as the Government reserves the right to make an award based on initial offers received, without discussion of such offers. E. Price Evaluation. Each offerors' price submitted in the Offer Schedule will be evaluated for completeness and reasonableness. Unrealistically low proposed costs/prices may be grounds for eliminating a proposal from competition either on the basis that either the offeror does not understand the requirement or the offeror has made an unrealistic proposal. Total Evaluated Price = (Total Evaluated Price for Base Period) + (Total Evaluated Price for Option Period 1) + (Total Evaluated Price for Option Period 2) + (Total Evaluated Price for Option Period 3) + (Total Evaluated Price for Option Period 4) + (Total Evaluated Price for Option to Extend Services IAW FAR 52.217-8). Total Evaluated Price for Base Period Option Periods 1 through 4: Quantities identified will be multiplied by the unit price to confirm the extended price and then the extended price for all CLINS will be added together for a total for that specific period. Unit price(s) shall be used in the event that extended price(s) are calculated incorrectly. Total Evaluated Price for Option to Extend Services IAW FAR 52.217-8: Offerors shall not submit pricing for the Option to Extend Services CLINS. The Total Evaluated Price for Option to Extend Services IAW FAR 52.217-8 will be calculated by multiplying a quantity of 3 by the unit price(s) proposed at Option Period 4. The price(s) are evaluated in the Total Evaluated Price for evaluation purposes only. The Option to Extend Services is not considered part of the contract period of performance and resultantly these totals shall not be included in the contract, however the CLINS will be added as information only. If the government exercises FAR 52.217-8, Option to Extend Services, the extension will not exceed 6 months total and the information only CLINS will be changed to priced CLINS at a unit price equal to the preceding period of performance's most current unit price. F. Technical Evaluation. Offers shall be evaluated independently of the cost/price proposal evaluation to determine the offerors compliance with the technical requirements of this solicitation. The government may make a final determination as to the rating of an offer based on the proposal as submitted without requesting any further information. The Technical factor will receive an overall rating of either "Acceptable," or "Unacceptable" using the rating and descriptions in Table 1 below: The Government's technical evaluation team shall evaluate the technical proposals on a pass/fail basis, assigning one of the ratings described below for each subfactor. Any subfactor evaluated as "Unacceptable" will render the entire proposal unacceptable and, therefore, un-awardable. Only those proposals determined to be technically acceptable, either initially or as a result of discussions, will be considered for award. However, the offeror is cautioned that the Government reserves the right to award this effort based on the initial proposal, as received, without discussion. Table 1. Technical Ratings Rating Description Acceptable Proposal clearly meets the minimum requirements of the solicitation. Unacceptable Proposal does not clearly meet the minimum requirements of the solicitation. Evaluators shall assign a rating, which falls within one of the following ranges according to the degree to which the offeror meets the minimum requirements described above: Technician Requirements in accordance with paragraph 3.0 of the SOW and solicitation Section A(3), Part III- Technical Proposal - Technical proposal will be rated acceptable if the offeror's submittal meets all requirements stated within these sections. G. The Government intends to evaluate offers and award a contract without discussions with offerors. Therefore, the offerors initial offer should contain the offerors best terms from a price and technical standpoint. However, the Government reserves the right to conduct discussions if later determined by the Contracting Officer to be necessary.
