Issued By: This Request for Information (RFI) is being issued by the Department of Veterans Affairs (VA) Program Contracting Activity Central (PCAC) for the Office of Informatics and Analytics (OIA) Health Informatics Interoperability Office (HIIO). RFI Purpose: This RFI is seeking written responses with information to help identify potential sources that are interested in and capable of performing the work described herein and as described in the draft Government Performance Work Statement (PWS). The Government is seeking to obtain industry comments and questions regarding a planned solicitation for HIG16-25 National Data Systems (NDS) Health Information Access (HIA) Services. This RFI requests responses from qualified firms. Submission Information: All potential sources with the capability to provide the requirements referenced in this RFI and in the draft Performance Work Statement (PWS) are invited to submit, in writing, sufficient information. This information must demonstrate the responder's ability to fulfill the requirements and be responsive to the technical questions in this RFI, as indicated below. Responses Due: All responses will be electronic format, and emailed to the Contracting Officer and the Contract Specialist. All responses must be received no later than Thursday, May 21, 2015, 4:00 PM Eastern Standard Time (EST). Submission Format: Submissions shall be in Microsoft Word or Adobe. Responses to this RFI must address the following requirements and questions: 1.A cover sheet which includes: a.RFI Title b.Vendor Company Name c.Address d.Name of Point of Contact e.E-Mail Address f.Phone Number g.Vendor DUNS Number h.Company business size and socioeconomic status (i.e., Large Business, Small Business, Service-Disabled Veteran Owned Small Business, Women-Owned Small Business,) the number of years in business, affiliate information: parent company, joint venture partners, potential teaming partners (if applicable) i.List of all North American Industry Classification System (NAICS) codes as identified in the vendor's Online Representations and Certifications Applications (SAM) j.If applicable, vendor's GSA and relevant SIN or other available Government-wide contract vehicle that is within the scope of this requirement 2.Past Performance - Please provide any contracts that your company has performed within the last three (3) years that are of comparable complexity and scope for requirement for which your company is responding, include the following: Please be specific and provide a complete reference to include at a minimum the project name (and the government contract number, if applicable), contract award amount, start and completion dates, and the project owners' contact information. Describe your Self-Performed* effort (as either a Prime or Sub-Contractor). Additionally, please provide the Contract Type: Firm Fixed-Price, Time and Material, etc. *Self-performed means work performed by the company themselves, NOT work performed by another company for them for any of the project examples provided. 3.Comments related to the Draft PWS and attachments. Please use track changes to identify questions, concerns, or comments within the attached documents. 4.Pricing Schedule - attached is a sample pricing schedule the government is considering for this action, we are requesting input on the viability of this structure or provide your input on suggested alternate price schedule. No pricing is requested. This is for research purposes. Additional Information: This RFI is issued solely for information and planning purposes and does not constitute a solicitation or obligation on the part of the Government. Neither unsolicited proposals nor any other kinds of offers will be considered in response to this RFI. No reimbursement will be made for any costs associated with providing information in response to this announcement and/or any follow-up information requests.
