Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6 -- Streamlined Procedures for Evaluation and Solicitation for Commercial Items, in conjunction with FAR -- Part 13 Simplified Acquisition Procedures; and supplemented with additional information included in this notice. This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-82, dated May 7, 2015. Solicitation VA262-15-Q-0539 is issued as a Request for Quote (RFQ) and constitutes the only solicitation. The Government intends to award a single Firm-Fixed Price (FFP) award with a base period of four (4) months and up to four (4) option years. This procurement will be issued as a Total Small Business Set-Aside. NAICS Code is 621910 and size standard is $15 Million. The Government is seeking a Contractor that can furnish 24-hour Basic Life Support (BLS) and Advanced Life Support (ALS) ambulance transportation service for the beneficiaries of the San Diego VA Medical Center. This will involve trips to and from any location within the Primary Service Area (PSA) of the San Diego VA Medical Center and beyond, as appropriate. The Contractor shall furnish all labor, vehicles, supplies, equipment and qualified personnel to perform the requested services. The Contractor shall also comply with all laws, policies, and/or regulations as required by local, state, and Federal agencies. See attached SF 1449 Solicitation for statement of work (SOW), vehicle/equipment list, and other pertinent information. Place of performances: VA San Diego Medical Center (VASDHS), 3350 La Jolla Village Drive, San Diego, CA 92161. Provisions 52.212-1, Instructions to Offerors - Commercial and 52.212-2, Evaluation - Commercial Items are applicable to this acquisition. Furthermore, Provision 52.212-2 is utilizing the Lowest Price Technically Acceptable (LPTA) on an Acceptable/Unacceptable evaluation process and award will be made to the offeror who conforms to the solicitation and represents best value to the Government.
