Description
This is a pre-solicitation notice to any vendors who can provide maintenance and support of the Paceart system. Vendors interested in bidding on this requirement must provide the following: Name of Company and address: P.O.C.: Business Size: (small or large, veteran owned status must be certified in Vetbiz, women owned, hub-zone etc ¦.) Must be registered in SAM (yes or no): Duns number: Tax ID number: Partial Statement of Work is cited below, adherence to the requirements is mandatory. The work will be done at the Lebanon VA Medical Center (LVAMC). The contract will be administered by the Cpl. Michael J. Crescenz Department of Veterans Affairs Medical Center. The period of performance for this requirement will have a base year (partial) and four option years. The solicitation may be issued May 15, 2015. The solicitation may be set aside for small business depending on the responses received. All responses and or inquiries must be emailed to Donna.Toliver3@va.gov due date for the responses to the pre-solicitation notice will be May 14, 2015. Partial SOW The Lebanon VA Medical Center (LVAMC) presently utilizes the Medtronic Paceart System to track data for cardiac devices. The LVAMC has a requirement for the maintenance and support of the Paceart system. Contractor shall provide maintenance and support services for all Paceart equipment, which includes the software, boards, and modules. Software covered includes network server image software, network image capture software, and advanced reporting software. Contractor shall provide all personnel, parts, service manuals, computer software and schematics necessary to provide on call technical support, update software when applicable, and effect repairs on Paceart software, modules, and boards. Contractor shall respond to calls from the LVAMC with a phone call to Biomedical Engineering (717-821-9015) within one (1) hour after receipt of initial telephoned notification, 24 hours per day. If the problem cannot be corrected by phone, the Contractor shall commence work (on-site physical response) within eight (8) normal duty hours after receipt of initial notification/service request, and shall proceed progressively to restore equipment to full operating performance in accordance with the conformance standards, within 48 hours of initial service request. The Vendor's service representative (VSR) shall contact Biomedical Engineering (717-821-9015) prior to responding to a service call which requires on-site repair or shipment of equipment/components. Contractor shall work with LVAMC's Biomedical Engineering to remediate any recalls and/or field service notices regarding equipment. The Contractor shall furnish and maintain in acceptable condition all calibration equipment to be used on LVAMC cardiology equipment. Prior to the commencement of work, the contractor shall provide calibration papers showing calibration was completed within last 12 months. Calibration shall be traceable to national standard. Contractor shall supply software updates as they come available without undue delay. If necessary, updates can be completed using remote connection, which shall follow VA standards. The Contractor shall furnish and replace parts. The Contractor shall have ready access to all parts, including unique and/or high mortality replacement parts. All parts supplied shall be compatible with existing equipment. The contractor shall use new original equipment manufacturer (OEM) parts. All parts shall be of current manufacture and have complete versatility with the presently installed equipment. All parts shall perform identically to the original equipment specifications. Rebuilt parts, used parts, or those removed from other equipment shall not be installed. To be eligible to perform under this contract, each contract employee shall have completed original equipment manufacturer (OEM) training within the last 36 months, have at least 24 months of experience working on equipment specified, and produce completion certificates to Contracting Officer Representative (COR) upon request. All formal contractor furnished training required shall be presented by persons who are certified or qualified to instruct the specific required subjects. Certification to instruct the specific subject shall be in the form of a certificate issued by an accredited institution of learning (school, college, university, etc.), a governmental agency (Federal, State, County, etc.), or by documentation that the person instructing has sufficient experience in the subject matter to be able to instruct the subject in an authoritative, practical and current manner. The VSRs shall be authorized by the Contractor to perform the maintenance services. All work shall be performed by "Fully Qualified" competent VSRs. The Contractor shall provide written assurance of the competency of their personnel and a list of credentials of approved FSEs for each make and model the Contractor services at the LVAMC. The Contracting Officer (CO) may authenticate the training requirements, request training certificates, or credentials from the Contractor at any time for any personnel who are servicing or installing any LVAMC equipment. The CO and/or COR specifically reserve the right to reject any of the Contractor's personnel and refuse them permission to work on the LVAMC equipment. If subcontractor(s) are used, they must be approved by the CO; the Contractor shall submit any proposed change in subcontractor(s) to the CO for approval/disapproval.
