This is a Limited Source Justification Notice. This is not a solicitation announcement. The purpose of this announcement is to state the results of the sources sought announcement and the determination that there is only one (1) responsible source that can currently provide the requested services within the Village Limits of Warsaw, NY 14569. This source is the Wyoming County Community Hospital. No other response to the sources sought notice had current phlebotomy services available at this time. Responses to this notice should be in writing and shall be faxed or emailed to 716-684-0474 or Sally.Gaerte2@va.gov. No telephone responses will be accepted. Responses must be received no later than 2:00 p.m. May 19, 2015.
