- J.4 Past Performance Questionnaire J.3 Compliance Program Attestation J.1 Consent to Subcontract Appendix A - CMS Assessment Data Element Library Governance Framework Attachment 2 - Section F SOD RFP-CMS-2015-150741 Attachment 1 - Section C SOW RFP-CMS-2015-150741 RFP-CMS-2015-150741 Section B - Section M SF-33 RFP-CMS-2015-150741 The Department of Health and Human Services (HHS), Centers for Medicare and Medicaid Services (CMS) intends to issue a Competitive Request for Proposal (RFP) for "Maintenance and Oversight Support of the CMS Data Element Library and the CMS Assessment Library Data Council". This program requires services to maintain and update CMS's publically available assessment data element Library, the CMS Data Element Library and to support the Library's oversight council, and the CMS Assessment Library Data Council. Ongoing support will be provided to CMS's electronically available, public facing data element Library to ensure that it is maintained with CMS's selected (up-to-date) data elements, directly supported as part of the maintenance process. All activities directed to maintenance activities include updating all relational mappings and procuring updated HIT Standards. The Contractor will ensure that the Library serves the public and CMS's purposes, and will comply with the requirements necessary for CMS to host the Library electronically in CMS's systems environment. Overall, the Contractor will support the Library as it continues in its evolution to support interoperable information exchange. CMS's goal is to enable and facilitate the adoption of health information technology and health information exchange, interoperability, across care providers through an accessible and up-to-date publically available repository that will serve as a resource housing CMS's data elements and their associated HIT standards. The functionalities of the Library are intentionally aimed at supporting interoperability. Additionally, in alignment with the IMPACT Act of 2014, this contract supports the use of EHR compatible, standardized data elements to enable longitudinal reporting of patient-centered quality outcomes across provider settings over time. This program is in the best interests of the Federal Government as this work is specified to include the ongoing maintenance of CMS's assessment-based data element inventory, and its associated relational data base, in accordance with CMS's standards and guidelines for the hosting of the electronic Library in the CMS environment. The services are to ensure that stakeholders have access to a viable product that meets both CMS's needs related to data element inventory, and the public's need for up-to-date standards and relational mappings. Further, this project is to obtain services to support the Library's oversight council, the CALDC. The Contractor will serve in the CMS Assessment Library Data Council's (CALDC's) "Governance Support Team" and "Librarian" roles. This procurement is a full and open small business competition, under NAICS code 541512 - Computer Systems Design Services, with a size standard $27.5 million. It is anticipated that CMS will award a one (1) year cost-plus-fixed-fee contract with four (4) option years. Contractors must be free of conflicts of interest or prepare a COI mitigation plan to address any actual, potential or apparent organizational conflicts of interest that could arise. The Government is not obligated or committed to award any contract as a result of this notice. Telephone and fax requests will not be honored. Contractors must be registered in the System for Award Management (SAM) database prior to award, during performance, and through final payment of any contract resulting from this RFP. Source selection procedures will be used with the intent to award a cost-plus-fixed-fee contract to the responsible offeror whose proposal, conforming to the Request for Proposal (RFP), is the most advantageous and offers the best value to the Government, price and other factors considered.
