Description
IMAT Workstation Statement of Work May 12, 2015 Solicitation No. DX00111Y2015T Combined Synopsis / Solicitation FEMA Workspace Transformation, Building 710, Denver Federal Center. GENERAL INFORMATION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format stated in the Federal Acquisition Regulations (FAR) subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only Solicitation. Quotations are being requested and a written solicitation will not be issued. The solicitation number is DX00111Y2015T. The solicitation is issued as a Request for Quotations (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through the Federal Acquisition Circular 2005-76. SET ASIDE: This solicitation and any subsequent award, is conducted as a small business-set aside. NAICS code: 442110; Small Business size standard 500 employees. LINE ITEMS: 1) Remove and dispose of existing non-systems furniture in four interior rooms (see attached SOW) 2) Furnish and install twelve (12) Teknion brand workstations, each with a three drawer filing cabinet (See attached SOW) 3) Provide ready to use, cleaned workstations, with removal and disposal of all packing, crating and otherwise unused or excess materials. DESCRIPTION OF REQUIREMENTS: The selected vendor shall provide all labor, material, equipment, supervision and staffing to remove and dispose of existing non-systems furniture from four interior rooms, and furnish and install twelve TEKNION brand work stations and file cabinets, per the attached Statement of Work, and drawing. (See attached SOW) SITE VISIT: Site visits to the work site will be conducted on an individual basis. Any interested offeror can schedule a site visit by contacting the point of contact as noted at the end of this document. Site visits will not be conducted on the date quotations are due. DATES: Date of Solicitation: May 12, 2015 Date Questions due: May 19, 2015 Date Quotations due: May 22, 2015 Date of Award (est.): May 28, 2015 Date of Completion: July 20, 2015 PLACE OF DELIVERY (FOB Point): FEMA, Building 710, Rooms 206, 207, 208 and 209, Denver Federal Center, Lakewood, Colorado, 80225. AWARD INFORMATION: FEMA intends the award of a firm fixed price purchase order, as a result of this combined synopsis/solicitation, based on price and delivery capability. Each offeror shall submit in their quotation, price, and a statement of delivery capability, per the requested date of completion. Price is considered as equally important as delivery. An award will be made to the offeror whose price is deemed to be realistic, Fair and Reasonable and which can most closely meet the completion date as stated. PAYMENT: Payment will be made via Electronic Funds Transfer (EFT) to the bank of record as stated in registration in the System for Award Management (SAM). A single payment, upon contract completion, is anticipated for this contract. Any partial payments must be approved in advance by the Contracting Officer. PROVISIONS: The provision at 52.212.1, Instructions to Offerors - Commercial, applies to this acquisition. Offerors must include a complete copy of the provision at 52.212-3, Offeror Representations and Certifications - Commercial Items, with their offer, or note that a current electronic copy is available at System for Award Management (SAM) at www.sam.gov. The clause at 52.212-4, Contract Terms and Conditions - Commercial Items, applies to this acquisition. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercials Items, applies to this acquisition. The following clauses from Clause 52.212-5 are applicable to this acquisition; 52.209-10, 52.233-3, 52.233-4, 52.219-6, 52.222-3, 52.222-19, 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.223-18, 52.232-33, SECURITY / SITE ACCESS REQUIREMENTS: All contractor personnel must be US citizens or US persons as defined by International Traffic in Arms Regulations and shall be cleared with FEMA Security. All contractor personnel will be subject to a favorable background investigation by the Department of Homeland Security. The appropriate security contact for the awarded contractor company shall submit a list of personnel to work on the contract to the FEMA Security representative, as provided at the time of award. That list shall include full name, date of birth, and Social Security number at least 72 hours prior to the scheduled work. SYSTEMS FOR AWARD MANAGEMENT REGISTRATION: All offerors wishing to be considered for this solicitation must be registered in the Systems for Award Management at www.sam.gov per Federal Acquisition Regulations section 4.11. Registration must be completed by the estimated award date. If registration is not completed, award will be made to the next eligible and qualified offeror. OFFER DUE DATE: The closing date for the receipt of RFQs is no later than 2 pm, MST, May 22, 2015. All offers shall be submitted via email to Joseph Gatlin at: joseph.gatlin@fema.dhs.gov. Fax submissions will not be accepted. Quotations shall reference the solicitation number DX00111Y2015T and shall be titled as "IMAT Workstations." Questions shall be submitted no later than 2 pm, MST, May 19, 2015. Questions shall be submitted to joseph.gatlin@fema.dhs.gov. Any questions submitted will be answered via an amendment to this synopsis/solicitation no later than May 20, 2015.
