Description

The Gulf Coast Veterans Health Care System (GCVHCS), Network Contracting Office (NCO) 16, Biloxi, MS, is issuing a combined synopsis/solicitation for commercial services for the Joint Ambulatory Care Center (JACC), Pensacola, FL. We intend to issue a sole source purchase order to Philips Healthcare, 22100 Bothell Everett Hwy, MS450, Bothell, WA 98021. This procurement is in accordance with FAR 6.302-1 (only one responsible source and no other supplies or services will satisfy agency requirements) for a one time service and repair to the Philips Mobile Fluoroscopy Unit. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation is being issued. The solicitation number VA256-15-Q-0649 is being issued as a Request for Quotation (RFQ). This document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-80 effective January 29, 2015. The NAICS code is 811219 and the size standard is $20.5 million. This requirement is full and open, sole source and will be firm-fixed-price. The closing date for this notice is 3:00 PM EST May 15, 2015. This procurement involves service, travel, parts and labor to repair the Model BV Pulsera fluoroscopy unit located at the JACC, Pensacola, FL. Contractor to supply only original equipment manufacturer parts, no refurbished, used or restocked parts will be accepted. Contractor to provide services in accordance with terms and conditions of the contract for the date of award through September 30, 2015. All work will be completed in accordance with the Scope of Work (see below). To be eligible for award of a contract, vendor must be actively registered in the SAM database (http://www.sam.gov). Vendor is also required to complete representations and certifications found in the provision at 52.212-3, Offeror Representations and Certifications - - Commercial Items. NO TELEPHONE REQUESTS OR INQUIRIES WILL BE ACCEPTED. Submissions must be received by 3:00 PM EST on May 15, 2015. Gulf Coast VA Medical Center Biloxi, MS 39531 SCOPE OF WORK AND GENERAL SPECIFICATIONS FOR PHILIPS MOBILE FLUOROSCOPY UNIT (C-ARM) MODEL BV PULSERA 1. Background The Gulf Coast VA Medical Center; Clinical Engineering Department provides comprehensive repair and service to all areas within the hospital and CBOCs for medical equipment. This includes all service and part orders necessary to bring medical equipment back to working condition. All equipment is utilized in patient care areas and used in diagnosing or treating our veteran patients. Without this equipment being repaired in a timely manner the hospital would need to fee out or cancel patient care throughout the facility. Due to this, all medical equipment orders for service and repair are considered a top priority and must be acted on expeditiously. 2. Specifications The Gulf Coast VAMC; Clinical Engineering Department specifications for this order are outlined below: "Contractor shall provide service to repair the following equipment: oPhilips Mobile Fluoroscopy Unit, Model BV Pulsera, Site ID# 41750637, EE # 46313. "Below are a list of all services needed: oTravel to and from facility oAll necessary parts/labor to repair ESU power assembly/DFI system to mfg. specifications "Contractor shall supply only OEM manufactured or approved new parts. No refurbished, used or re-stocked parts will be sent. "Point of contact is Albert Celestino and can be reached at 850-912-2237. "Contractor shall report to the Biomed Shop, Room 2B151 at Joint Ambulatory Care Center, 790 Veterans Way, Pensacola, FL 32507, to sign in before making service call. "Contractor shall provide equipment specific vendor trained individuals to service the equipment. Contractor shall provide all documentation/certificates showing completion of equipment specific training from the vendor by the individual conducting the repair. oShall be produced, reviewed, and approved by the COR before award. "Any contractor (individual completing repair) other than the OEM manufacturer must have completed no less than four (4) service repairs on equipment (stated in the SOW) within the last calendar year. For each instance they shall provide the following: equipment name, equipment model, date, repair description. "The contractor shall provide a service ticket for the maintenance service at the completion of the work. This ticket must show the service performed, parts used, and signature of employee completing work. "The contractor shall ship all parts and provide service in the most expeditious manner available. "Any part or service not covered by this service contract will not be completed without first procuring a formal purchase order from the VAMC, Engineering Service, Biloxi, MS 39531. "The contractor shall obtain all necessary licenses and/or permits required to perform this work. Contractor shall take all precautions necessary to protect persons and property from damage during performance of this contract. Contractor shall be responsible for any injury to himself or his employees, or others, as well as for any damage to personal property that occurs during the performance of this contract that is caused by him/her or his/her employee's fault or negligence. The contractor shall maintain personal liability and property damage insurance prescribed by the laws of the State of Florida. "Contractor shall provide services during normal work hours of 8:00 a.m. to 4:30 p.m. Monday through Friday, except Federal Holidays. Federal Holidays: New Year's Day, Martin Luther King Day, Presidents Day, Memorial Day, Independence Day, Labor Day, Columbus Day, Veterans Day, Thanksgiving Day, Christmas Day, and any other day specifically declared a by the President of the United States to be a federal holiday. "The contractor, their personnel, and their subcontractors shall be subject to the Federal laws, regulations, standards, and VA Directives and Handbooks regarding information and information system security as delineated in this contract. "Incorporate the security clause (Appendix B) into the contract involved and the appropriate security/privacy language outlined in Appendix C into the solicitation. "Contractor shall complete the repair for the Philips BV Pulsera within 7 days of PO issuance and/or contract award. 3.
