*** INFORMATION ONLY *** Oklahoma City VA Medical Center Oklahoma City OK 73104 Scope of Work Background: The Oklahoma City VA Medical Center (OKC OK) is looking for a company to repair, maintain, and/or service approximately 128 Multifunctional Devices (MFD)/Copiers. Project Scope: The Oklahoma City VA Medical Center needs to put in place a service contract to repair and maintain Ricoh MFD/Copier machines, model# C4501 4 each, model# MP5001G 52 each, and model# MP2851SP 72 each. Service calls placed shall be responded to by an OEM factory trained technician, within 2 hours by phone and must be onsite with an average response time of 4 hours. The contractor must repair or replace all the parts of Ricoh multifunctional devices/copiers to include software, firmware, and hardware with OEM parts only. Once it is determined that a part needs to be replaced, that part and replacement of that part shall be made within 5 business days bringing that equipment back into an operational status. Service Locations: VAMC in Oklahoma City, OK (majority of MFD located in OKC) Outpatient Clinic Ardmore, OK Outpatient Clinic Stillwater, OK Outpatient Clinic Fort Sill, OK IMPORTANT NOTE: This is for requesting information only no award will be made off of this RFI. Please communicate via email to Lance.Davis2@va.gov by COB May 18, 2015 as to your company's ability to perform service per this reduced Performance Work Statement. Please also provide the size of your company (Large Business, Small Business, VOSB, or SDVOSB) (along with DUNS number).
