Description
RFQ-MA-15-00006 Attachment 3, Amendment 1, Field Measurement Accreditation Attachment 1, Amendment 1 Statement of Work Amendment 1 revises the Line Items, Statement of Work Tasks and Period of Performance. The Line Items are changed from the Base and Two Options to a Single Period of Performance (Base) with no options. Line Items 0002 and 0003 are deleted and line item 0001 is revised to include all required tasks for this effort. The period of performance for this effort shall be proposed by the offeror, but shall not exceed 5 years. The anticipated start date is corrected from 06/01/2016 to 06/15/2015. Please see the revised Attachment 1, Statement of Work and Attachment 3, Scope of Field Measurement Accreditation, which have been revised to be consistent with the change to one line item. Description of the Requirement: As described in Attachment 1, Statement of Work, the United States Environmental Protection Agency (EPA) requires an Accrediting Body (AB) to implement ISO/IEC 17025 requirements for: 1)Continued accreditation of our fixed laboratory location in North Chelmsford, MA as a testing laboratory in environmental chemistry for current ISO/IEC 17025:2005 fields of accreditation, target analytes, and methods shown in Attachment 2. (This list may be subject to changes in method, but the overall scope will not expand.) 2)Gap Analysis and subsequent phased accreditation of field sampling and measurement activities for fields of activity, and methods shown in Attachment 3. This is a solicitation for commercial items prepared in accordance with the format in Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a request for quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-81. This is a total small business set aside. Vendors must be registered in the System for Award Management (http://sam.gov) under NAICS code 813920, Professional Organizations, size standard $14,000,000.00, to be considered for award under this solicitation. If you are not already registered under this code and meet the size standard, you must add this NAICS code to your SAM.gov registration. Item Number: 0001 Audit for ISO/IEC 17025 Accreditation for EPA Region 1 Laboratory The Contractor shall provide ISO/IEC 17025 Fixed Laboratory Accreditation and Field Sampling and Measurement Accreditation as required in the attached Statement of Work (Attachment 1) for the EPA Region 1 Laboratory located at: 11 Technology Drive North Chelmsford, MA. 01863 The overall length of the period of performance for this line item will be proposed by the offeror, but shall not exceed 5 years. The anticipated start date is 06/15/2015. This line item is firm fixed priced: To Be Proposed Quote Instructions: a. Please provide a firm fixed price quote for the services identified in Line Item 0001. The price shall include all costs associated with the required service to include all labor, materials and travel. The offeror may use their standard commercial format for submittal of their price quote. b. Please include a brief description of your approach to meeting the requirements of each task. Limit this description to 1 page for each task. c. Please provide at least one reference for a similar effort, but no more than three references. Include a name, phone number and e-mail address for each reference. d. Please include documentation to show your recognition as a signatory to the International Laboratory Accreditation Cooperation (ILAC) Mutual Recognition Agreement (MRA.) e. Please include your DUNS number with your quote. f. Offerors shall comply with instructions identified in provision at 52.212-3, Offeror Representations and Certifications-Commercial Items, for completion of Representations and Certifications. The Offeror shall complete only paragraph (b) of this provision if the Offeror has completed the annual representations and certification electronically via the System for Award Management (SAM) Web site accessed through http://www.acquisition.gov, or http://sam.gov. If the Offeror has not completed the annual representations and certifications electronically, the Offeror shall complete only paragraphs (c) through (p) of this provision. g. Offerors are also required to complete the provision at EPA-K-04-101, Representation by Corporations Regarding a Felony Conviction under Federal Law or Unpaid Federal Tax Liability., which can be found on page 29 of the Provisions and Clauses. Please check off is or is not for questions (b) (1) and (2). Award Criteria: In accordance with FAR 13.106-2 an award will be made to the offeror who can provide the best value. When determining best value the following evaluation factors will be considered along with price: past performance and compliance with all requirements as stated in the RFQ. Evaluation factors other than price when combined are approximately equal to price. Offerors who do not provide all of the requested information will not be considered for award. The Contracting Officer may contact offerors to request clarification of information provided to better understand your quote or may request information that appears to be inadvertently omitted. Submittal Instructions: Quotes are due May 29, 2015 by 4:00 PM EDT. Quotes shall be e-mailed to Patrice Denault at denault.patrice@epa.gov by the deadline. Quotes received after the required date and time will not be considered for award. Please contact the Contracting Officer, Patrice Denault, if you have any questions. All questions must be received by May 20, 2015.
