- Statement of Work (SOW) - ALL areas must be met to qualify for award Request for Quote (RFQ) Packet - Please fill out all areas specified in the notes on page 2 and return by end date 05/21/2015. U.S. Customs and Border Protection has an immediate requirement to establish a Public Access Defibrillation (PAD) Program in accordance with the specifications listed in the attached Statement of Work (SOW). The purpose of this procurement action is to establish an agreement with a responsible contractor to conduct a site assessment and review of current AED placement in CBP facilities throughout the Buffalo Field Office, review current program and response protocols, and to develop the Buffalo Field Office PAD program to meet or exceed Federal guidelines, including training of volunteer CBP AED responders. Additionally, the selected contractor shall be capable of handling the majority of the administrative functions (including equipment monitoring, upkeep and replacement) necessary to ensure program continuity and minimize the amount of effort required by CBP staff to coordinate and administer the Buffalo Field Office PAD program. Please see attached SOW for further details of requirement. This is a Request for Quotes (RFQ) for the period of performance of July 1, 2015 - June 30, 2016. During this time, the Contractor shall provide oversight of the Buffalo Field Office PAD program to meet or exceed Federal guidelines, including ongoing training and recertification of between 150 and 200 volunteer AED responders. Training for employees at the CBP Area Ports specified in SOW shall be scheduled between October 1, 2015 - April 30, 2016, and coordinated with the CBP facilities via the Buffalo Field Office CBP AED/CPR Program Manager. Award of this requirement will be subject to availability of Fiscal Year 2015 funds, and will be made on the basis of lowest-priced and technically acceptable quote in compliance with the SOW. The vendor selected for award MUST register in System for Award Management (SAM) at https://www.sam.gov/portal/public/SAM/ and receive payment via Electronic Funds Transfer (EFT) as a condition of the contract. Registration in SAM requires a valid Dun & Bradstreet (DUNS) number. No award can be made to a company not registered in SAM. Please also complete and sign Blocks 13, 14, 15 and 16 on page one of this SF 18 form. Quotes that are incomplete, unsigned, or late will not be considered. Delivery of Submittals: Both Quote and first two (2) pages of the RFQ packet must be RECEIVED through email or Fax by the Contract Specialist no later than May 21, 2015 at 5:00 P.M. Eastern Time (Indianapolis). No phone calls will be accepted. Late submissions will not be accepted.
