- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation VA259-15-Q-0412 is issued as a request for proposal (RFP). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-82, effective May 07, 2015. No telephone requests will be accepted. Only written requests received directly from the offeror are acceptable. Contractor shall ensure that all employees present a clean, neat and professional appearance. The Fisher House is a Home Away from Home (Hotel-Like Facility) where patients and family members who live far away, can stay while receiving ongoing medical treatment. The contract will consist of a base year with four one year options. Service shall be required five (5) days per week (Monday - Friday); service shall include all common areas, guest rooms/bathrooms, storage rooms, front and rear patios and office areas. Cleaning of guest rooms shall be performed after each guest's departure. Note: during their stay, Fisher House guests are responsible for cleaning their own bedrooms/bathrooms as needed and therefore occupied guest rooms shall not require cleaning by the contractor. The contractor shall be responsible for exchanging new laundry for guests' old laundry and processing all linens to get them ready for laundry pickup. This includes looking for stains and spot treating before laundry pick up as well as sorting clean linens when they arrive from the laundry. This shall include articles such as bath towels, wash cloths, shower curtains, sheets, pillowcases, duvet covers, pillow shams, blankets, comforters, mattress pad covers & pillows. The Fisher House consists of 20 private bedrooms and bathrooms and is approximately 14,000 square feet. SCOPE OF WORK: All work shall be performed by skilled and trained personnel who communicate fluent English. The contractor shall provide all necessary labor, travel, supervision, and coordination. The contractor shall provide one (1) worker every Monday through Friday from 10:00 a.m. to 6:30 p.m. (8 hour workday) to perform housekeeping services at the Fisher House. The company shall ensure there is always a trained backup person available to cover when the regular employee is out sick, on vacation or otherwise unavailable. The backup person shall be able to report to work within two hours of being notified. Typical work shall include, but not be limited to vacuuming, mopping, bathroom cleaning, kitchen cleaning, window cleaning, dusting, taking out trash/recycle, changing light bulbs, etc. (see frequency chart below). The worker shall be required to log in and out at the Fisher House office for the beginning and end of shift, providing date and time, respectively, in a log. The worker shall be sharing duties with the VA housekeeper who works until about 4:00 p.m., and the contractor shall then be on their own until 6:30 p.m. It is expected that all tasks shall be performed in a neat, professional manner that presents a good appearance. Any storage area shall be maintained in a neat, clean and orderly manner. The contractor shall be fully responsible for any repairs or damage to VA property which results from the activities, negligence, or carelessness of its employees or subcontractors. All damages shall be immediately reported to the point of contact (POC) and Fisher House manager by the end of the workday of occurrences. If damage is not discovered on the day the work is performed, it shall be repaired within 72 hours of notification of the contractor by the VA. In cases of disputes between the POC and the contractor, the contractor shall notify the contracting officer, in writing as soon as possible to avoid any delay in conflict resolution. If the contracting officer is not notified in writing within 5 days of the date of the incident, the incident shall be considered to have been resolved to the satisfaction of the contractor. Before commencing work under the contract, the contractor shall provide to the contracting officer a certificate of insurance indicating sufficient coverage outlined herein and containing an endorsement to the effect that cancellation of, or any material change in the policy which adversely affects the interests of the Government in such insurance shall not be effective unless a 30-day advance written notice of cancellation or change provided to the Government. The VA will appoint a POC to monitor performance to ensure services are received. The POC will evaluate the contractors' performance through intermittent on-site inspections of the contractor's quality control program and receipt and investigation of complaints from personnel. The VA may inspect each task as completed. The VA shall make final determination of the validity of customer complaint(s) in cases of disagreement with customer(s). Ten days prior to the implementation of this contract, contractor shall provide the POC with written copies of procedures to be used to carry out the requirements of the contract. Performance Objective: Custodial Services - not to exceed two (2) customer complaints per month. Contract Manager: The contractor shall provide a contract manager who shall be responsible for the performance of the contract. The contract manager shall have full authority to act on behalf of the contractor on all contract matters relating to the daily operation of this contract. The contract manager shall be able to read, write, speak, and understand English. The contract manager or alternate(s) shall be available during normal business hours and be able to meet, either in person or by telephone, with VA personnel designated by the Contracting Officer within 24 hours to discuss deficiencies. Contractor personnel shall present a neat appearance and be easily recognized as contract employees. This may be accomplished by wearing distinctive clothing provided by the Fisher House, or by clothing bearing the company name and/or wearing appropriate badges that prominently display the contractor's company name and the employee name(s). Jeans, shorts and sandals are not considered appropriate or safe attire for this setting. Contractor shall have experience in hospitality and cleaning in residential or hotel like settings. They shall be able to communicate with guests staying in the Fisher House hospitably and understand they are serving the guests during their stay. The contractor shall not employ persons for work on this contract if such an employee is identified to the contractor by VA police or contracting officer as a potential threat to the health, safety, security, general wellbeing, or mission of the medical center, VA or Fisher House. The contractor shall not employ any person who is presently an employee of the U.S. Government when and if the employment of that person would create either a conflict of interest or the perception of such a conflict. Contractor personnel shall report to the Fisher House Manager or designee at the beginning of each workday to coordinate the areas to be completed that day. Work shall be performed in 8-hour shifts, generally between 10:00 a.m. and 6:30 p.m. Monday through Friday. Absences shall be reported to the contractor who shall notify Fisher House Staff. The contractor shall provide back-up staffing when needed. Logging in and out is required when entering or leaving the Fisher House. Work required in the performance of the contract shall be performed during the hours of 10:00 a.m. to 6:30 p.m. Monday through Friday, including Federal Holidays. This shall be defined as straight time. Non-normal duty hours are defined as any other hours outside of those defined above. The following holidays are observed by this facility: Any holidays falling on Saturday will be observed on the proceeding Friday; holidays falling on Sunday will be observed on the following Monday. The contractor shall assure that the assigned staff members are qualified and able to provide excellent customer service including the ability to work with the VA housekeeping staff. Contractor shall be subject to frequent unannounced safety and environmental inspections. Contractor shall have five (5) working days to correct sanitation infractions designated as non-critical and two (2) working days to correct critical infractions. Orientation and initial training of all personnel shall be accomplished within the first fourteen (14) days of employment with written documentation furnished to the POC. All training shall be documented and written copies of signed attendance sheet provided to POC within ten (10) days of completion of training. New personnel, hired during the contract period of performance shall be trained and oriented within this time frame, so there is always a backup available. Contract personnel shall complete documentation per day on what cleaning tasks were accomplished using worksheets Fisher House will assign. (c) Definitions: The following definitions are provided to assist evaluators in the evaluation of each non-cost/price factor. 1) Strength: An aspect of an offeror's proposal that has merit or exceeds specified performance or capability requirements in a way that will be advantageous to the Government during contract performance. 2). Weakness: A flaw in the proposal that increases the risk of unsuccessful contract performance. 3). Deficiency: A material failure of a proposal to meet a Government requirement or a combination of significant weaknesses in a proposal that increases the risk of unsuccessful contract performance to an unacceptable level. (d) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. These rates may be adjusted only as a result of revisions to prevailing labor rates provided by the Secretary of Labor. The option provision may be exercised more than once, but the total extension of performance hereunder shall not exceed 6 months. The Contracting Officer may exercise the option by written notice to the Contractor within 30 days. 52.217-9 OPTION TO EXTEND THE TERM OF THE CONTRACT (MAR 2000) (a) The Government may extend the term of this contract by written notice to the Contractor within 30 days; provided that the Government gives the Contractor a preliminary written notice of its intent to extend at least 60 days before the contract expires. The preliminary notice does not commit the Government to an extension. (b) If the Government exercises this option, the extended contract shall be considered to include this option clause. (c) The total duration of this contract, including the exercise of any options under this clause, shall not exceed 5 years. (End of Clause) FAR 52.228-5 Insurance - Work on a Government Installation (JAN 1997); CL-120 Supplemental Insurance Requirements ; In accordance with FAR 28.307-2 and FAR 52.228-5, the following minimum coverage shall apply to this contract: (a) Workers' compensation and employers liability: Contractors are required to comply with applicable Federal and State workers' compensation and occupational disease statutes. If occupational diseases are not compensable under those statutes, they shall be covered under the employer's liability section of the insurance policy, except when contract operations are so commingled with a Contractor's commercial operations that it would not be practical to require this coverage. Employer's liability coverage of at least $100,000 is required, except in States with exclusive or monopolistic funds that do not permit workers' compensation to be written by private carriers. (b) General Liability: $1,000,000.00 per occurrences. $20,000.00 property damage. (d) The successful bidder must present to the Contracting Officer, prior to award, evidence of general liability insurance without any exclusionary clauses for asbestos that would void the general liability coverage. End of Clause FAR 52.232-19 AVAILABILITY OF APPROPRIATED FUNDS FOR THE NEXT FISCAL YEAR (APR 1984) Funds are not presently available for performance under this contract beyond September 30 of ANY fiscal year. The Government's obligation for performance of this contract beyond that date is contingent upon the availability of appropriated funds from which payment for contract purposes can be made. No legal liability on the part of the Government for any payment may arise for performance under this contract beyond September 30 of ANY fiscal year, until funds are made available to the Contracting Officer for performance and until the Contractor receives notice of availability, to be confirmed in writing by the Contracting Officer. (End of Clause) FAR 52.232-40 PROVIDING ACCELERATED PAYMENTS TOSMALL BUSINESS SUBCONTRACTORS (DEC 2013); FAR 52.237-3 CONTINUITY OF SERVICES (JAN 1991); VAAR 852.203-70 COMMERCIAL ADVERTISING (JAN 2008); LIMITATIONS ON SUBCONTRACTING-- MONITORING AND COMPLIANCE (JUN 2011): This solicitation includes FAR 52.219-6 VA Notice of Total Small Business Set-Aside (Dec 2009). Accordingly, any contract resulting from this solicitation will include this clause. The contractor is advised in performing contract administration functions, the CO may use the services of a support contractor(s) retained by VA to assist in assessing the contractor's compliance with the limitations on subcontracting or percentage of work performance requirements specified in the clause. To that end, the support contractor(s) may require access to contractor's offices where the contractor's business records or other proprietary data are retained and to review such business records regarding the contractor's compliance with this requirement. All support contractors conducting this review on behalf of VA will be required to sign an "Information Protection and Non-Disclosure and Disclosure of Conflicts of Interest Agreement" to ensure the contractor's business records or other proprietary data reviewed or obtained in the course of assisting the CO in assessing the contractor for compliance are protected to ensure information or data is not improperly disclosed or other impropriety occurs.
