- SCA Wage Determination Required Vendor Forms Contains Scope of Work, Bid Sheet, Terms & Conditions, and other essential information THIS IS A SUBCONTRACTING OPPORTUNITY with Management & Training Corporation (MTC), operator of the Hawaii Job Corps Center, referred to henceforth as the Center. The Center is requesting bids for the services as referenced in the Statement of Work. Please provide a price quote on the attached Bid Sheet by no later than 4:00 PM (Hawaii Time) on Friday, June 5, 2015. Statement of Work: 1) Contractor shall provide CPR & First Aid Training and Certification for the Center's staff and trainees a. Those successfully completing the course shall be issued official certification from either the American Heart Association or American Red Cross i. Certification shall be in the form of a card, to be provided to Center for each recipient within 10 days of course completion 2) Potential frequency of training courses needed is estimated as follows, but is not a guarantee nor a binding commitment: a. Courses per Year: 22 - 26 b. Frequency: Every other Friday during regular business days c. Group Size: 8 - 15 students per course session d. Estimates are for the training needs of our trainees only. Staff training needs will vary. 3) All courses shall be conducted on-site at the Center 4) Contractor shall provide all necessary textbooks, supplies, and equipment a. Center has 4-6 CPR manikins ("annies") available for Contractors use, as a courtesy. Center is not obligated to furnish any supplies or equipment 5) Contractor shall possess valid insurance and be certified as instructor by the American Red Cross or American Heart Association at all times during contract period 6) Contractor shall invoice Center per course a. A written list of all participants, indicating completion or non-completion of each participant, must be submitted with invoice It is the Contractor's responsibility to verify the Scope of Work or conduct any necessary site visits prior to submitting a bid.
