- Draft PWS for Data to Care Technical Assistance. For Sources Sought Purposes Only THIS IS NOT A SOLICITATION, REQUEST FOR PROPOSAL (RFP), INVITATION FOR BID (IFB), REQUEST FOR QUOTE (RFQ), OR REQUEST FOR APPLICATION OF A SOLICITATION, AND DOES NOT ASSUME THE GOVERNMENT WILL AWARD A CONTRACT OR PROCEED WITH ANY OF THE ABOVE SOLICITATIONS IN THE NEAR FUTURE. THIS NOTICE IS A SMALL BUSINESS ONLY SOURCES SOUGHT REQUEST FOR MARKET RESEARCH PURPOSES. This program is intended to provide technical assistance to state and local public health jurisdictions throughout the United States on how to use HIV surveillance data to identify HIV-positive diagnosed individuals not in care, link them to care, and support the HIV care continuum. Goals: 1) To build the capacity of health department jurisdictions, through targeted technical assistance, in using their HIV surveillance data to identify HIV-positive diagnosed individuals not in care and link them to care 2) To reduce the number of new HIV infections in the United States The core tasks for the contract will include: 1) Collaborate with CDC staff to provide Data to Care technical assistance within selected jurisdictions; 2) Develop an operational and management work plan to provide Data to Care TA to the selected state and local jurisdictions; 3) Design Data to Care products, resources and tools needed to develop and support a Data to Care program 4) Provide targeted Data to Care technical assistance to jurisdictions throughout the US; 5) Provide a summary report of all technical assistance activities; 6) Transfer Data to Care knowledge, skills and resources acquired during the contract to CDC's Data to Care-designated Capacity Building Assistance providers via training and technical assistance. Contractor Capabilities must include 1) The Contractor shall have demonstrated successful experience with providing Data to Care technical assistance services. 2) The Contractor shall be familiar with CDC HIV Prevention with Positives (PWP) guidelines. 3) The Contractor shall be familiar with the HIV continuum of care as well as strategies for working with persons living with HIV. 4) The Contractor shall be familiar with the challenges, including structural barriers that impact HIV linkage, engagement and retention in care and viral suppression for persons living with HIV. 5) The contractor shall be familiar with the National HIV/AIDS strategy as well as the CDC's Division of HIV/AIDS Prevention's High Impact Prevention strategy. 6) The Contractor shall have experience working with or in state and local health departments on topics related to HIV surveillance, prevention and/or care programs (e.g., data to care program models, data security, data sharing). 7) The Contractor shall be able to travel to the CDC headquarters in Atlanta, GA, to the jurisdictions selected for participation in the program, and the sites selected for Data to Care Capacity Building Assistance providers CAPABILITY STATEMENTS MUST DEMONSTRATE THE MINIMUM REQUIREMENTS OUTLINED ABOVE. Capability statements, not to exceed seven pages in length, shall include the following information: company name, address, point of contact, phone/fax/email, and business size and status (e.g. small business, 8(a), veteran-owned small business, service-disabled veteran owned small business, HUBZone small business, small disadvantage business, and women owned small business). The NAICS code for this requirement is 541690. Small Businesses having the capabilities necessary to perform the stated requirements may submit capability statement via email to ins4@cdc.gov. The subject line of the email shall be "Comprehensive Wellness Capability Statement." Responses are due not later than June 2, 2015. Capability statements will not be accepted after the due date. A rough draft version of a possible Performance Work Statement (PWS) is attached to this Sources Sought Notification. This draft is only intended as a guide in assisting sources in viewing the possible magnitude of the project. Disclaimer and Important Notes. The Government reserves the right to use information provided by respondents for any purpose deemed necessary and legally appropriate. Any organization responding to this notice should ensure that its response is complete and sufficiently detailed to allow the Government to determine the organization's qualifications to perform the work. Respondents are advised that the Government is under no obligation to acknowledge receipt of the information received or provide feedback to respondents with respect to any information submitted. After a review of the responses received, a pre-solicitation synopsis and solicitation may be published in Federal Business Opportunities. However, responses to this notice will not be considered adequate responses to a solicitation. Confidentiality. No proprietary, classified, confidential, or sensitive information should be included in your response. The Government reserves the right to use any non-proprietary technical information in any resultant solicitation(s).
