Description: COMMUNITY BASED OUTPATIENT CLINIC VA244-15-R-0478 The Erie Veterans Affairs Medical Center is soliciting to lease 6200 - 6800 net useable square foot of office/clinical space to operate a primary care clinic in Warren County, PA This space is for use as a Community Based Outpatient Clinic (CBOC) by the Erie VA Medical Center, Erie, PA. All space must meet the functional goals of Patient Aligned Care Team (PACT) model and commercial best practice for clinical operations, and must meet all Government requirements for fire safety and ADA accessibility. On-site parking for a minimum of 50 vehicles and of these, six (6) spaces must be provided immediately adjacent to the building's main entrances for use by the handicapped (HP) The Government will consider both existing and build to suit properties. A lease for a base period of five (5) years with a one - five (5) year option period for a total of 10 years will be considered. Space is required to be located within the Warren County Pennsylvania area, specifically within the following geographical service area and delineated boundaries: Geographical service area Ś City/Town of Ś Warren, PA Boundary description Ś No more than 5 miles out, North, South, East and West from the City Limits of Warren, PA. Additionally, the boundaries encompass the commercial district of Warren City/County for convenient accessibility of veterans served in the community. Interested Lessor's are expected to submit a complete and comprehensive proposal construction and design schedule that describes all phases of the project within 14 days of award. (see 2.3 of the solicitation) Duration of design and construction is 240 days from award. The government anticipates award before 1 August 2015. Proposals are due on or before 6/26/2015 at 4pm e.s.t.
