Description
COMMERCIAL MAT RENTAL and CLEANING SERVICES Jack C. Montgomery Veterans Affairs Medical Center (JCMVAMC) Veterans Health Administration (VHA) Department of Veterans Affairs (VA) This is a solicitation for commercial services, to include commercial items, prepared in accordance with the format in FAR Part 15 and FAR Subpart 12.6 as supplemented with additional information included in this notice. This solicitation is issued as a Request for Quote (RFQ) and the reference number is listed on page one (1) of this solicitation. This is not a request for proposal and the government does not intend on holding negotiations. The Veterans Affairs Medical Center requires Commercial Mat Rental and Cleaning Services with the intent of producing clean, safe and professional walkway mats throughout the assigned areas of each Location, considering VA Medical Center visitors and staff, ensuring cost effectiveness, whilst in support of the VA Mission. Refer to the solicitation and Performance Work Statement for details and instructions. Locations: Jack C. Montgomery VA Medical Center (JCMVAMC) Address: 1011 Honor Heights Drive Muskogee, OK 74401 Buildings at JCMVAMC: 1, 6, 7, 8, 9, 10, 11, 18, 22, 24, 28, 53 Ernest Childers VA Outpatient Clinic (ECOPC) Address: 9322 East 41st St Tulsa, OK 74145 Period of Performance: This contract is for a Base Year, with four (4) one (1) year options. Base Year- Date of Award through May 31, 2016 Option Year 1- June 01, 2016 through May 31, 2017 Option Year 2- June 01, 2017 through May 31, 2018 Option Year 3- June 01, 2018 through May 31, 2019 Option Year 4- June 01, 2019 through May 31, 2020 (Option Years pending availability of funds. Refer to FAR clause 52.217-8 and 52.217-9) This is 100 percent set aside for small business. The NAICS Code and the size standard are listed on page one (1) of this solicitation. Written offers are required (oral offers will not be accepted). Submission of offers must be in accordance with FAR 52.212-1, Instructions to Offerors - Commercial Items. This is a Best Value procurement. The Government will award a firm-fixed price contract resulting from this solicitation to the responsible Offeror submitting an overall offer that is determined most advantageous to the Government, price and non-price factors considered. Refer to Evaluation Factors and Information to Offerors. When: Offers are due per the time and date indicated on page one (1) of this solicitation. How: Offers are to be submitted electronically by email. Complete Offers shall be submitted as follows: 1)Emailed to the Solicitation POC 2)Include subject line: "Company Name, solicitation number" 3)Indicate the number of emails submitted as part of the offer 4)Notify the Solicitation POC at least in advance of the Due Date/Time if a posted or faxed offer submission is preferred. Posted or faxed offer submissions must be marked for: "Company Name, solicitation number" and attention to the Solicitation POC Documented tracking and confirmation of email delivery is the responsibility of each Offeror. Email Size: Each email is not to exceed 5mb each. For other requirements What to Submit: A Complete Offer will consist of one (1) original of the following documents. 1)Page 1 of this solicitation completed (this is preferred in lieu of an offer cover letter) and must include: a.date and signature by a representative of the company authorized to submit the offer, b.the offer expiration date, c.Offeror's confirmation of availability to start service (pending contract), d.Offeror's DUNS number or ORCA application status, and e.Offeror's business size 2)Vendor's acknowledgement of any solicitation amendments using Section B of the solicitation 3)Unit prices and extended prices for each line Item included in the Price/Cost Schedule of this solicitation; 4)Technical Capability and Past Performance Late or Non-responsive Offers: It is the responsibility of each Offeror to ensure their offer is delivered Complete, on time and in full, per this solicitation. The VA Contracting Specialist will review the offers received for completeness, and may reject any incomplete offers. Queries and Questions: These must be addressed in writing to the Solicitation POC. Telephone requests for the solicitation will not be accepted or honored. Telephone questions and answers regarding this solicitation will not be considered valid. Technical questions will only be answered when addressed in writing and only when submitted to the RFQ POC. Technical questions are due one (1) business day prior to the Closing Date and Time indicated on page 1 of the solicitation. SITE VISIT: A Site Visit may be arranged for the sole purpose of viewing MAT locations at JCMVAMC. A Site Visit may be arranged by writing to the POC. At no time whatsoever, will any information provided during a Site Visit, and by anyone other than the Solicitation POC, be valid in relation to the RFQ and pending Contract.
