Attachment J.5 Experience Questionnaire Attachment J.4 Gila National Forest Fire Plan Attachment J.3 Wage Rates SF-1442 Solicitation, Offer And Award (Must Submit With Offer) Attachment J.2 Drawings for Black Canyon Tributary Bridge 150 Attachment J.2 Drawings for East Indian Creek Bridge Attachment J.1Special Contract Requirements/Section C/Black Canyon Tributary Bridge Attachment J.1 Special Contract Requirements/Section C/East Indian Creek Bridge Solicitation 2015 EAST INDIAN CREEK BRIDGE/BLACK CANYON TRIBUTARY BRIDGE 150 The Gila National Forest has a requirement for the replacement of two bridges on NFSR 150; East Indian Creek Bridge and Black Canyon Tributary Bridge at Hodge Canyon. Work includes but is not limited to removal and disposal of existing bridge abutments and superstructure; and furnishing and placement of approach rails and bridge rails. Contractor shall furnish all materials, equipment, labor, transportation and supplies necessary to complete this project.

Hwy 35 for approximately 15.1 miles; then North on NFSR 150 for approximtately 21.6 miles to Black Canyon Tributary and continue for an additional 23.1 miles East Indian Creek Bridge., Silver City, New Mexico, 88061, United States
