- STATEMENT OF WORK Repair and Extend Cut Fiber Optic Cable Purpose: Provide all labor, materials, equipment, tools, supervision and other appurtenances required for the complete repair, extension and testing of a cut 12-strand fiber optic cable. Project Location: Building No. 5 VA Medical Center 1111 East End Boulevard Wilkes-Barre, PA 18711. Description of Work: The fiber optic cable is a 12-strand cable and is located in the basement/crawlspace. The cable must be spliced, extended and terminated from the point of repair to the existing data closet which will require approximately 100 of new FO Cable. Contractor shall repair fiber using a 12-strand SM/MM Fiber Splice Tray with Wall Mount Housing. The fiber shall be installed in a minimum 1" inner duct plenum. Cable shall be fully terminated in the data closet using a 12-strand fan-out kit and 62.5 MM fusion splice connectors. Cable shall be tested in accordance with FOA Standards FOA-1, 2, 3 and 4. The Contractor shall provide the VA with a certificate indicating that the cable was tested and met or exceeded all testing requirements. All work shall be performed in a neat and workmanlike manner and shall comply with all FOA Standards for both installation and testing of Fiber Optic Cable. Permits: 1.Contractor shall obtain and pay for all required permits for performing the work. Time of Operations: 1.Contractor may perform work during normal clinic operating hours (M-F 7:30 AM to 4:30 PM) or after hours and on weekends if a representative of the Owner/Lessor is available to escort and provide access. The VA shall not provide after-hours or weekend escort. This Request for Quotation is open to small businesses only. Please note: 852.215-70 Service-Disabled Veteran-Owned and Veteran-Owned Small Business Evaluation Factors. As prescribed in 815.304-71(a), insert the following clause: SERVICE-DISABLED VETERAN-OWNED AND VETERAN-OWNED SMALL BUSINESS EVALUATION FACTORS (DEC 2009) (a) In an effort to achieve socioeconomic small business goals, depending on the evaluation factors included in the solicitation, VA shall evaluate offerors based on their service-disabled veteran-owned or veteran-owned small business status and their proposed use of eligible service-disabled veteran-owned small businesses and veteran-owned small businesses as subcontractors.
