Synopsis Announcement: 20 May 2015 Subject: Fare Free Mass (FFM) and Permanent Change of Station (PCS) Shuttle Bus Services for United States Forces Korea (USFK) Attachment: PWS Subject: This is a Synopsis of FFM and PCS Shuttle Bus Services. Solicitation of the requirement will be posted in ASFI in a few weeks. Description: This is a synopsis for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement serves to inform solicitation will be issued. The 411th Contracting Support Brigade (CSB) Korea intends to award FFM and PCS Shuttle Bus Services for USFK. This requirement is commercial items that the contractor shall provide all necessary personnel, buses, equipment, supplies, transportation, tools, materials, supervision, and other items and non-personal service necessary to perform shuttle bus service to USFK personnel and family members. All required materials, labor, equipment and technology are available locally and Invited Contractors (IC) Status under the Status of Forces Agreement (SOFA) will not be granted. GENERAL: This is a contract to provide all buses, operators, fuel, facilities and management required to perform the shuttle bus including PCS (Permanent Change-of Station) bus services at the areas designated in the contract, in accordance with the Performance Work Statement (PWS). 1.1 Description of Services/Introduction: The contractor shall provide all personnel, equipment, supplies, facilities, transportation, tools, materials, supervision, and other items and non-personal services necessary to perform Fare-Free Mass Transit (FFMT) and shuttle bus services and PCS bus services as defined in this PWS to support ground transportations to US Army personnel and family members PCSing into the Republic of Korea (ROK) by US Government Air or personnel in leave or TDY status except for those items specified as government furnished property and services. 1.2. Scope: 1.2.1. The contractor shall provide all buses, operators, fuel, facilities and management required to perform the bus services at the areas designated in the contract, in accordance with the PWS The contractor shall perform the requirements in this PWS and conform to the professional standards identified in this contract. The Contractor shall follow all applicable instructions and directives to meet the requirements for FFMT and shuttle bus services throughout United States Forces Korea (USFK) as specified Exhibit 1 thru Exhibit 36. 1.2.2. The scope of PCS bus services in this contract is to provide commercial bus service for movement of passengers traveling in PCS status and accompanying baggage for USFK wide (Yongsan and Tongduchon area), from Inchon International Airport to Seoul Local Hotels Dragon Hill Lodge (DHL), Capital, Hamilton, Ambassador, Crown, Tower, Rainbow, Itaewon, Olympic Parktel, and other hotels in Seoul area) seven days a week. PCS travelers have first priority and TDY travelers also have priority. All other travelers are on a space available basis. 1.3. Background: This contract is designed for the performance of the shuttle bus and PCS bus services in accordance with (IAW) all terms and condition of this contract. 1.4. Period of Performance: The effective period of this contract is a one-year base performance period plus four (4) one-year option performance periods. Base Year: 1 January 2016 - 31 December 2016 1st Option Year: 1 January 2017 - 31 December 2017 2nd Option Year: 1 January 2018 - 31 December 2018 3rd Option Year: 1 January 2019 - 31 December 2019 4th Option Year: 1 January 2020 - 31 December 2020 1.5. Type of Contract: The Government contemplates award with hybrid Firm Fixed Price (FFP) and Requirements type contract. 1.6. Quality Control Plan: The contractor shall perform the maintenance and repair of buses used in the performance of this contract to ensure that the buses operate safely at all times. All maintenance, other than operator maintenance, shall be performed at the contractor's facility. The contractor shall also adhere to the required bus schedules and routes. To meet these requirements, the contractor shall develop a Quality Control Plan to include: 1.6.1. A bus maintenance schedule to include the frequency and type of maintenance to be performed. The contractor shall verify that the maintenance type and frequency is performed to the vehicle manufacturer's standard. 1.6.2. Maintenance and repair history for the previous 12 months for each bus proposed to be used in the performance of the contract. If buses are new, submit planned and scheduled maintenance for the upcoming 12 months for each bus proposed to be used in performance of this contract. 1.6. 3. A back-up plan to provide replacement buses within two (2) hours if buses breakdown during schedule runs. 1.6. 4. A plan to conduct random inspections relative to all aspects of contract performance (i.e., adherence to bus schedules; interior cleaning of buses, adherence to inspection, maintenance and repair of buses; conduct of drivers). 1.6. 5. A plan to receive and resolve customer complaints. 1.6. 6. The contractor shall meet with the COR as needed but a minimum once per month to discuss contract performance and resolve any problems, which may exist. 1.6. 7. In case of an emergency the contractor shall contact the Provost Marshal at the nearest appropriate installation. 1.6.8. The contractor shall keep records of inspections and make the records available to the Government throughout the PoP for the contract and for the period after contract completion until the final settlement of any claims under this contract. The contractor shall collect data, perform self-evaluation, and document performance against standards associated with each specific requirement. The contractor shall report by the 7th calendar day of the previous month the inspection results in the Contractor Quality Control Inspections sheets. The contractor shall address all tasks in the PWS based on the example below. Quality Inspection And Assurance Plan: The Government may conduct on-site inspections at a minimum Included in the inspection will be a review of all required permits and insurance documents, as well as maintenance records. The buses will also be inspected by US Government-employed mechanics for any safety or maintenance violations. 1.8. Buses: 1.8.1. General: The contractor shall obtain all appropriate permits, such as bus business license and applicable route permits from the Ministry of Land, Transport and Maritime Affairs (MLTM) in Republic of Korea (ROK). The company shall provide, within two (2) hours, replacement transportation for buses which experience mechanical breakdown with passengers aboard. The contractor shall post bus schedules at all bus stops. 1.8.2. Minimum Standards for Buses: 1.8.2.1. All buses furnished by the contractor shall be passenger type (commercial design) with seating capacity of 41 - 45. The buses provided by the contractor shall be equipped with spotlights, which provide sufficient lighting to facilitate safe entry and exit of passengers. Buses shall be in a safe and serviceable condition at all times, and shall not exceed the life expectancy of 7 years of age. Buses shall be equipped with devices and appliances for passenger comfort in conformance with common commercial practice as well as operational and safety requirements of AR 385-55 and USFK Reg. 190-1 as cited in Appendix II. The buses shall be equipped with Anti-Lock Braking Systems (ABS). 1.8.2.2. In providing PCS bus services under the terms of this contract, the Contractor shall provide passenger buses with seating capacity of 41 - 45 and equipped with on-board video monitor and VCR for purpose of displaying Korean orientation and related welcome videos for arriving soldiers. Buses shall be also provided maximum storage area for high volume of personal baggage. For Inchon: Each bus shall be equipped with a cell phone, or a compatible means of communication, for direct contact with 1 RC Movement NCOs located in the Inchon passenger terminal. The Contractor shall provide a phone number with tag/plate number for each bus to 1RC Movement NCOs while the drivers are waiting at the Inchon International Airport. 1RC Movement NCOs will contact drivers when arriving personnel are ready to be picked up. 1RC Movement NCOs will also provide specific location for personnel pick up. 1.8.2.3. Seating area, overhead luggage rack, and floors of the buses shall be dry and free of debris at all times. 1.8.2.4. Identification of route (s) shall be clearly posted while buses are in operation. Customer complaint procedures shall be posted on the bus stops and terminals. 1.8.2.5. During the period 15 May through 15 September all buses used in the performance of this contract shall be air-conditioned. 1.8.2.6. Buses shall be equipped with an appropriate heating system during the winter season, from 16 September to 14 May. 1.8.2.7. Maintenance: Buses shall be equipped with a serviceable fire extinguisher, First Aid Kit and Triangles at all times. 1.8.2.8. Bus maintenance, other than operator maintenance shall be performed at contractor's facility. 1.9. Insurance Liabilities: 1.9.1. The Contractor shall provide and maintain insurance for the contractor-operated buses with ROK accredited insurance company as follows: 1.9.2. Group-type bodily injury and death liability insurance in an amount of not less than $200,000.00 on one person and not less than $500,000.00 on one accident or occurrence. 1.9.3. Property damage liability Insurance in an amount of not less than $100,000.00 on account for one accident. 1.9.4. The contractor shall submit the proof of insurance coverage of to the Contracting Officer within five (5) calendar days after award of the contract. 1.10. Management Plan: The contractor shall have a management plan to operate the required 36 bus routes plus additional back-up buses and sufficient bus drivers to meet the requirement of this contract. Bus List: The Contractor shall have bus list showing the buses to be used for the performance of this contract. Original copies of all Valid Vehicle Registration Certificates, Valid Auto Insurance and other Insurance Policies shall be submitted to the COR for review within the five (5) calendar days after award of the contract and the COR will return the Original copies to the contractor after review. 1.11. Contractor Qualification: 1.11.1. Prime Contractor's Prior Experience: The contractor shall have at least two (2) years experience within the last seven (7) years performing in Shuttle Bus services. The experience of a subcontractor cannot be substituted for the prime contractor's experience. 1.11.2. Registration: The Prime Contractor shall possess and submit the following valid and current certification, registrations and licenses issued by Republic of Korea (ROK) Government. (a)Chartered Bus Business Registration ( ). (b)Passenger Transportation Business Registration ( ). (c)Chartered Bus Transportation Business Registration ( ). (d)Vehicle Maintenance Management Registration (Comprehensive Maintenance) [ ( )] (e)Designated Motor Vehicles Maintenance Certificate (Comprehensive) [ ( )] 1.12. Personnel/Key Personnel. 1.12.1. Definition: quote mark Key Personnel quote mark mean employees of the Contractor, or any subcontractor(s), affiliates, joint venture partners, or team members, and consultants engaged by any of those entities that are considered to be essential to the work being performed under this contract. Key Personnel are as follows: Contract Manager 1.12.2. Before removing, replacing, or diverting any of the listed or specified key personnel, the Contractor shall: (1) Submit notification to the Administrative Contracting Officer (ACO) reasonably in advance (14 calendar days is considered reasonable); (2) Submit justification (including proposed substitutions with resumes of equal qualifications stated in this section 1.11.) in sufficient detail to permit evaluation of the impact on this contract; and (3) Obtain the Contracting Officer's written approval. Notwithstanding the foregoing, if the Contractor deems immediate removal or suspension of any member of its management team is necessary to fulfill its obligation to maintain satisfactory standards of employee competency, conduct, and integrity under the clause at 48 CFR 970.5203-3, Contractor's Organization, the Contractor may remove or suspend such person at once, although the Contractor shall notify the Contracting Officer prior to or concurrently with such action. 1.12.3. The list of key personnel may, with the consent of the contracting parties, be amended from time to time during the course of the contract to add or delete personnel so long as the aforementioned items are addressed. 1.12.3.1. Contract Manager (CM): 1.12.3.1.1. The Contractor shall provide a CM who has at least two (2) years experience as a CM with bus transportation operations and services within last seven (7) years and who is knowledgeable of both US and Korean applicable transportation regulations and directive. 1.12.3.1.2. The CM shall have the ability to speak, read and write in English. 1.12.3.1.3. The CM shall provide contact information where he or she can be reached in the event of an emergency. The CM shall contact the COR within 10 minutes of the occurrence of the emergency (such as an accident, a typhoon, unexpected). 1.12.3.2. Bus Drivers: 1.12.3.2.1. Each driver employed by the contractor to provide the required bus service shall have in their possession at all times a valid Class 1 Bus driver's license as required by the Government of the Republic of Korea. 1.12.3.2.2. The contractor shall submit a list of all drivers who shall be performing services, along with evidence of their qualifications before award for a technical evaluation. 1.12.3.2.3. The contractor shall provide a list of all drivers with valid identification for all contract personnel requiring installation passes & decals within five (5) calendar days after award to a COR. The COR will, in turn, obtain the required signatory authority and provide the installation passes to the contractor for distribution to appropriate contract personnel. The processing time for USFK passes and decals will take minimum thirty (30) working days. 1.12.3.2.4. Driver Performance. The contractor shall comply with all applicable safety laws, rules, ordinances, and regulations promulgated by the Korean Government when operating outside military installations or areas controlled by the U.S. Government. When operating within the confines of a U.S. Government area, all rules and regulations as determined by the United States Forces Korea shall apply. (See Appendix II) 1.12.3.2.5. Driver's shall maintain a well-groomed appearance and wear contractor-provided nametags in English at all times while operating buses. 1.12.3.2.6. Bus drivers shall not operate buses with standing passengers when traveling off military installations. It shall be the driver's responsibility to assure that the number of passengers transported in each bus does not exceed the designed seating capacity. Buses operating solely within the confines of a military installation may allow one-half of the bus seating capacity to stand. 1.12.3.2.7. Drivers shall operate buses in strict adherence to traffic signs posted within all military installations. 1.12.3.2.8. Buses are not authorized to stop for refuel while en route. The bus driver shall re-fuel before dispatching. 1.12.3.2.9. Pets are authorized on the bus as long as the pet is in a pet carrier. 1.12.3.2.10. All vehicle operators shall ensure their vehicles are locked and properly secured when not in use. Key shall not left in any unattended buses. 1.13. Subcontracting: The Prime Contractor shall perform on the site, and with its own organization and equipment, work equivalent to at least 60 percent of the total amount of work to be performed under the contract. This percentage may be reduced by a supplemental agreement to this contract if, during performing the work, the Contractor requests a reduction and the Contracting Officer determines that the reduction would be to the advantage of the Government. 1.14. Physical Security: 1.16.1. The Contractor and its employees must comply with all security requirements imposed by the local installation commander at all times while performing under this contract within the military installation. 1.16.2. Personnel Pass and Escort Passes: It shall be the Contractor's responsibility to submit the appropriate documents for personnel and escort passes that is required for entry onto the installation to the requiring activity. 1.17. Safety Plan. 1.17.1. The Contractor shall be responsible for ensuring that all work performed under this contract is accomplished in a safe and proper manner. 1.17.2. The Contractor shall develop and maintain a safety plan that describes all safety requirements for this project. Due to safety requirements, the number of passengers transported shall not exceed the seating capacity of the bus off military installations. 1.17.3 The safety plan shall be submitted with initial proposal as part of the technical proposal. 1.17.4. All Contractors' employees shall be instructed and trained on accident prevention and safety by the project manager. Special emphasis shall be placed on the safe operation of equipment and proper working conditions. Training shall be conducted for all employees by the Contractor on appropriate safety measures and on the employee's obligation to comply with all existing installation safety regulations. This training shall be documented and provided to the COR. 1.17.5. All Contractor equipment shall be equipped with proper safety devises and in safe operating condition at all times. All contractor equipment shall be subject to a safety inspection at any time by the COR. Any defective equipment shall either be repaired or replaced. 1.17.6. The Contractor shall park the buses at the specified locations IAW Appendix III. 1.18. Post Award Conference/Periodic Progress Meetings: The Contractor agrees to attend any post award conference convened by the contracting activity or contract administration office in accordance with Federal Acquisition Regulation Subpart 42.5. The contracting officer, Contracting Officers Representative (COR), and other Government personnel, as appropriate, may meet periodically with the contractor to review the contractor's performance. At these meetings the contracting officer will apprise the contractor of how the government views the contractor's performance and the contractor shall apprise the Government of problems, if any, being experienced. Appropriate action shall be taken to resolve outstanding issues. These meetings shall be at no additional cost to the government. 2. DEFINITIONS AND ACRONYMS: 2.1. Definitions: Contracting Officer: A person with the authority to enter into, administer, and/or terminate contracts and make related determinations and findings. Contracting Officer's Representative (COR): A US Government employee selected and designated in writing by the contracting officer to act as his/her designated in writing by the contracting officer to act as his/her designated representative in administering a contract. Government: Government of the United States of America unless specifically identified as the Government of the Republic of Korea. Operator Maintenance: Bus maintenance performed by the bus driver which is limited to checking fluid levels, adding fluids, changing tires, and washing the vehicle. Quality Control: Those actions taken by a contractor to control the performance of services to ensure that they meet the requirements of the contract. Quality Assessment and Improvement: Those actions taken by the government to check services to determine if they meet the requirements, terms and conditions of the contract. 2.2. GOVERNMENT FURNISHED ITEMS AND SERVICES: 3.1. The Government will provide parking and operator maintenance space for the contractor's buses to include electricity, water and heating fuel at no cost to the contractor. (See Appendix III) 3.2 The contractor shall be responsible for the cleanliness of all areas provided to the contractor by the Government. 3.3. The Government will provide a telephone, however, the contractor shall have responsible for any cost associate or incurred through the use of the telephone. 3.4. Equipment: The Government will provide Global Positioning Systems (GPS) for vehicle tracking, at no expense to the contractor when it becomes available. 4. CONTRACTOR FURNISHED ITEMS AND RESPONSIBILITIES: 4.1. The Contractor shall perform appropriate inspections and maintenance for the safe operation of all buses furnished under this contract prior to every run. Such maintenance shall include cleanliness of buses, and internal area of the buses shall be dry and free of debris at all times. Buses shall be broom swept prior to spotting thereof for loading of passengers, seats shall be dry wiped and buses shall be washed internally and externally as required. All buses shall be equipped with sufficient lighting inside the bus to provide safe entry and exit of passengers when the buses are operated during hours of darkness. All buses shall be also equipped with heating system during winter season. Air-conditioned buses shall be used during summer season and will be provided at no additional cost to the Government. 4.2. The contractor shall provide nametags to be worn by all contract personnel while engaged in the performance of this contract. 4.3. The contractor shall post a sign inside each bus to read quote mark Senior Military Occupant is Responsible for Maintaining Order in this Bus. quote mark In the event a passenger becomes unruly, belligerent, or fails to comply with the provisions of this agreement, the senior military occupant in the bus shall assist the driver to maintain order. The contractor shall notify the Military Police and the transportation representative of the nearest installation serviced by the agreement of such incidents as soon as possible. Once notified, the Military Police and the transportation representative will take appropriate corrective action. Problems relating to such incidents shall be resolved between the transportation representative and the contractor. 4.4. The contractor shall not indemnify for lost or damaged items, luggage or parcels accompanying bus patrons. 4.5. The Contractor shall post a sign in each bus in English and Hangul to read as follows: No Smoking. Playing of radio/cassette/CD players not allowed (except with personal earphones). Attached POC phone number in each bus. 4.6. Notice of Accident: All cases of personal injury involving death or requiring medical attention of passengers in the performance of this contract shall be verbally reported immediately by the Contractor to the nearest MCT from the accident site. A full and complete written report of the accident shall be submitted to the Contracting Officer, 411th Contracting Support Brigade, APO AP 96218-5682, through the COR, within 72 hours following the verbal report. 4.7. Items Left on Bus By Passengers: Any items left on the bus by passengers and discovered by the Contractor shall be recorded and turned over to the receiving officer, the nearest MCT, or Contracting Officer Representative (COR). 5. SPECIFIC TASKS: 5.1. Shuttle Bus Services: (Requirement Portion) 5.1.1. Performance Requirements: The Contractor shall perform services of transporting passengers under this contract in accordance with procedures set forth below: 5.1.1.1. Commitment: For Shuttle bus services under this contract, Tasking of the contractor's service shall be made when Movement Control Team (MCT) requests to bus service to the Ordering Officer for the US Government. Commitment will be made orally or in writing by the Ordering Officer to the contractor's designated representative. 5.1.1.2. Tasking Procedure: Tasking of the Contractor's services shall be made no later than (NLT) 1600 hours of the day prior to the operation. A duly appointed Ordering Officer shall task the contractor's bus services. The Ordering Officer is authorized to place orders in this contract not to exceed KW25,000,000 per delivery order. It is responsibility of the contractor to ensure that a tasking is only made by Ordering Officer who is appointed by Contracting Officer (KO). The Ordering Officer may not make any changes in the terms and conditions of any contract which the Ordering Officer places orders. 411th CSB will officially notify the contractor about any changes that were suggested and approved by the KO. 5.1.1.3. MCT will provide the Transportation Movement Release (TMR) numbers and consignee, location where passengers is to be picked up. 5.1.1.4. Cancellation Notice: When any changes for the planned mission schedule or cancellation have to be made, a written or telephone notification will be provided no later than 1700 hours, on the day before the commitment. However, in case of emergency circumstances, such as alert, adverse weather conditions or other unavoidable circumstances, the Contractor will be notified as soon as feasibly possible. 5.1.1.4.1. Cancellation Charges: When and if buses are actually requested as ordered by the Ordering Officer and cancelled the cancellation charges shall be paid to the contractor. 5.1.1.5. Detention (Waiting) Charges: Detention charges will be paid to the contractor when and if the buses are detained at the pick up point or the destination as follows. 5.1.1.5.1. Free of Charge will be one (1) hour from the spotting time at pick up points and one (1) hour after arrival at the destination. 5.1.1.6. Overnight charges: Overnight charges will be authorized to the contractor when the bus cannot return to the overnight designated parking area by midnight (24:00) of the same day. 5.2. PCS Bus Services: 5.2.1. Commitment of Service: Normally, commitment of the Contractor's service shall be made by the Ordering Officer, 662nd MCT and 584th DST (Uijongbu), 25th Transportation Battalion, in writing using EATC Form 35 before 1000 hrs on the performance day. In the event of an emergency, however, commitment may be issued verbally by the Ordering Officer with notice as short as three hours prior to spotting time at the origin point, in which case the verbal commitment shall be confirmed in writing by the Ordering Officer within 24 hours following the verbal commitment. 5.2.2. Delays/Cancellation of order: 5.2.2.1. Delay: In case of flight delay, the Government will notify the contractor via Fax or E-mail 3 hours prior to spot time of the performance date without additional cost to the Government. 5.2.2.2. Cancellation: In case of cancellation by the Government, the Government will notify the fact to the contractor one day prior to the performance date without additional cost to the Government. 5.2.3. Dispatch of Buses: The Contractor shall insure that all buses are dispatched on time at the designated locations. 5.3. Service Operating Procedures. 5.3.1. Buses committed by the Ordering Officer shall report at the point of origin at the specified spotting time. The Contractor shall, at this time, present a Contract Bus Transportation Service Certificate, EATC Form 35, issued by the Ordering Officer that prepares the passenger movement commitment to the responsible agent of 662nd MCT and 584th DST (Uijongbu). 5.3.2. When loading of passengers is completed, the buses shall be released for movement by returning EATC Form 35 to the drivers after annotating the date required by 25th Trans Bn. 5.3.3. Buses shall be operated in a professional and courteous manner in conformance with the best commercial practice and local rules and regulations. It shall be the driver's responsibility to ensure that the number of passengers does not exceed the seating capacity of the buses and that only persons authorized by the U.S. Government are transported. Drivers shall obey the Republic of Korea law and USFK regulation. Drivers shall not operate buses in such a manner that will jeopardize the safety of the passengers. Drivers shall operate buses in strict adherence to road signs as posted within all U.S. military installations. 5.3.4. Baggage: 5.3.4.1. When the bus is full, the U.S. Government is responsible for transporting the rest baggage. 5.3.4.2. When a commitment calls for one bus and the number of passengers to be transported is 20 or less, all baggage belonging to the passengers shall be moved in the same bus. In this case, the Contractor shall provide the means of protection against hazards for passengers and baggage. 5.3.5. Documentation: 5.3.5.1. Contract Bus Transportation Service Certificate. In committing and verifying the Contractor's service, Contract Bus Transportation Service Certificate, EATC Form 35, shall be used. The preparation and distribution of the certificate shall be made as follows: 5.3.5.2. Preparation - EATC Form 35 shall be prepared by the Ordering Officer in three (3) copies entering items in Section I. Entries to the certificate shall be made as follows: Section I: By the Issuing Officer Section II: By the Shipping Officer Section III: By the Receiving Officer Section IV: By the Shipping Officer (Round trip only) Section V: By the Receiving Officer (Round trip only) Section VI: By the Contractor 5.4. Payment: Government Bill Of Lading (GBL) USFK Form 79-3 EK 5.4.1. Contractor will receive an order for the bus service under this contract by means of a properly executed U.S. Government Bill of Lading, USFK Form 79-3EK. In the event the original Bill of Lading is lost or destroyed, a Certificate in Lieu of Lost U.S. Government Bill of Lading, USFK Form 79-6EK, shall be used to document. 5.4.2. When ready for mission, the contractor shall report to the Ordering Officer at the origin MCT to receive necessary transportation documents Government Bill of Lading (GBL). 5.4.3. Contractor shall at all times attach the US Government Bill of Lading with delivery ticket. Contractor shall use the delivery ticket as documentation, which is the primary document to issue GBL when commercial transportation is used. 5.4.4. Contractor shall distribute the US Government Bill of Lading and Bill for Accessorial Services as described below. US Government Bill of Lading Distribution: The issued GBL with Invoices submit to 25th Trans BN and 19th ESC. After reviewing and verifying by COR, submit to the J4 for ROK MND Logistics Bureau. 5. 5. The contractor shall maintain buses in safe operating conditions and perform regularly schedule maintenance. The contractor shall verify that the maintenance type and frequency is performed to the vehicle manufacturer's standard. Maintenance shall include, but not be limited to, braking system, steering system, electrical and lighting system, tires and tire pressure, hydraulics, fuel lines, and fluids. 6. APPLICABLE DOCUMENTS: The following documents form a part of this Performance Work Statement and will be in effect on the date of Request for Proposals. 6.1. USFK Regulation 55-37 Transportation and Travel - KOREA TRAFFIC MANAGEMENT, dated 12 August 2013. 6.2. Mileage Distance Table (2008), published by the 25th Transportation Battalion (25th TB), Unit #15181, APO AP 96218-5181, in effect at the time of commitment of services. 6.3. The following table identifies the performance objectives considered essential for the contracted services to be performed in a satisfactory manner. It is the Contractor's responsibility to establish an effective quality control system to ensure all performance standards identified below are met or exceeded. Any performance objective evaluated by COR and found unsatisfactory will be considered as an unsatisfactory performance for the inspection purposes even if re-inspected at a later date if it is outside of the established acceptable quality level. Any performance objectives below the performance standards identified for the surveillance period will be documented as unsatisfactory performance. 9.2. Inspection Rating Procedures. When performance is determined acceptable, the COR will document the acceptable performance rating in the inspection form (USFK 173-R, Contractor's Performance Rating). When the performance is determined unacceptable, the COR will inform the Contractor's on site representative that the performance is unacceptable, state the reason why, and request re-performance of services if appropriate. The COR will document the unacceptable performance in USFK Form 173-R, Contractor's Performance Rating, or DA Form 5479-R, Contractor's Discrepancy Report if applicable, and reasons why and requested re-performance completion date. The COR will request the contract manager initial the surveillance form. If the contactor requests to challenge the results of the surveillance, the COR will refer them to the Contracting Officer for resolution. 9.3. Unacceptable Performance. When re-performance is determined unacceptable or has not been completed, the COR will inform the Contractor's on-site representative that performance is still unacceptable, state the reasons and request their initials and date on the USFK 173-R. If the contractor requests to challenge the results of the surveillance, the COR will refer them to the Contractor Officer for resolution. CONTRACTOR MANAGEMENT REPORTING (CMR): The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the Contractor shall report ALL Contractor manpower (including subcontractor manpower) required for performance of this contract. The Contractor shall completely fill in all the information in the format using the following web address https://Contractormanpower.army.pentagon.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical Representative (COTR) or also known as the Contracting Officer's Representative (COR); (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by reporting period; (4) Contractor's name, address, phone number, e-mail address, identity of Contractor employee entering data; (5) Estimated direct labor hours (including sub-Contractors); (6) Estimated direct labor dollars paid this reporting period (including sub-Contractors); (7) Total payments (including sub-Contractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by Contractor (and separate predominant FSC for each sub-Contractor if different); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requiring Activity is responsible for providing the Contractor with its UIC for the purposes of reporting this information); (11) Locations where Contractor and sub-Contractors perform the work (specified by zip code in the United States and nearest city, country, when in an overseas location, using standardized nomenclature provided on website); (12) Presence of deployment or contingency contract language; and (13) Number of Contractor and sub-Contractor employees deployed in theater this reporting period (by country). As part of its submission, the Contractor shall provide the estimated total cost (if any) incurred to comply with this reporting requirement. Reporting period shall be the period of performance not to exceed 12 months ending September 30 of each government fiscal year and must be reported by 31 October of each calendar year. Contractors may use a direct XML data transfer to the database server or fill in the fields on the website. The XML direct transfer is a format for transferring files from a Contractor's system to the secure website without the need for separate data entries for each required data element at the website.
