VENDOR SUBMITTAL CHECKLIST m643p SAMPLE CCV SAMPLE Type 7 SAMPLE 643U SAMPLE 448 SAMPLE 428 SAMPLE 346 SAMPLE 326 SAMPLE Water Tender Solicitation Items to be Awarded BASE BPA SOLICITATION DOCUMENT This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information attached to this notice. The USDA Forest Service intends to procure four (4) Tactical Water Tender Type 22 Fire Apparatus in conjunction with establishing additional Blanket Purchase Agreements (BPA's) with vendors interested in supplying off vehicle body fire apparatus equipment. Vendors selected for award will also be included on the BPA list to fill future orders for the Forest Service on a national basis, and may be utilized by other designated Federal Agencies. The national standardized specifications and supplemental specification documents for the apparatus are separate attachments within this notice. This solicitation is set-aside 100% for small business concerns. The applicable North America Industry Classification System (NAICS) code for this acquisition is 336211- Motor Vehicle Body Manufacturing. The small business size standard is 1,000 employees. The contractor is responsible for providing all labor, supervision, tools, materials, equipment, and other resources necessary to manufacture the fire apparatus, mount it on the government furnished cab and chassis, and deliver it to the designated location. Transportation is f.o.b. destination. The fully assembled apparatus shall be delivered to the multiple locations outlined in the Schedule of Items within the specified period of performance. The period of performance will be 120 days from the date of award. To be considered for award, offerors must comply with all of the terms and conditions of the contract, including the specifications, delivery schedule, and warranty provisions. If any modification to the terms and conditions is proposed, offerors shall clearly describe the changes and include, if appropriate, descriptive materials such as illustrations or drawings to aid in understanding the changes. The selected offerors must comply with all clauses and provisions incorporated in this contract. All responsible sources that submit an offer will be considered. Offerors should include past performance information with their quotes to aid the contracting officer in determining responsibility. Past performance information includes experience in work of a similar type and scope, including contract numbers, project titles, dollar amounts, and the names and phone numbers of points of contact for the contracting entity. Offers will be evaluated on price and conformance with contract requirements.
