- FAR 52.212-3 Offeror Representations and Certifications Attachment Statement of Work Combined Synopsis/Solicitation Amendment: 22 May 2015 This amendment will include changes to the Combined Synopsis/Solicitation and Statement of Work. The change summary has been included below. Change to Combined Synopsis/Solicitation: A more flexible delivery/installation timeframe has been made. It has been changed from 30 days to 90 days. Please see Page 2 for the new delivery/installation timeframe; also stated here: 'All deliveries and installation shall be accomplished no later than 90 days after receipt of award.' Additionally, there has been an addition in the Addenda to FAR 52.212-1 Instructions to Offerors - Commercial Items. This addition can be found within 'Quote Specifics' on Page 2, paragraph B, #4. It states, 'If offerors provide a quote utilizing a consolidation point, then a second quote for commercial UK FOB Destination delivery is required.' Change to Statement of Work (specifically Attachment 1, Requirements Listing): Page 9 - The requiring activity no longer needs furniture for the Mental Health and Alcohol and Drug Abuse Prevention and Treatment (ADAPT) waiting rooms. As a result, these quantities have been deleted from Attachment 1. Page 9 - The requiring activity has identified the need for 12 single chairs for the OR. As a result, this has now been added. The resulting changes have reduced the total quantity of single chairs required from 86 to 62.
