Scope of Work for Project ID# NH2015 - 0001 Driving instruction program for students New Hampshire Corps Center 943 Dunbarton Road Manchester, NH 03102 Operated by Adams and Associates, Inc. Mailing Address: New Hampshire Job Corps Center 943 Dunbarton Road Manchester, NH 03102 Site Visit: July 15, 2015 Proposal Due Date: August 17, 2015 Submission of Proposals: New Hampshire Job Corps Center Attn: Ruber Rodriguez rrodriguez@adamsaai.com The only Bids that will be considered will be from vendors who attend the mandatory bid conference (Site Visit) on the day and time specified above. Project Summary: To provide a minimum of: • (30) hours of classroom instructions with a certified driver education instructor. • (10) hours of behind-the-wheel practice with a certified driver education instructor. • ( 6 ) hours of driving observation with a certified driver education instructor, • Evaluation of students in order to schedule road test. a) Contractor must visit the site and familiarize himself with the condition under which the work will be performed. No allowance will be made for any error on the contractors part for failure to take into account any conditions or circumstance existing at the site which might affect the contemplate work. The fact that a bid is submitted with ID # NH2015-0001 will be taken as evidence that the contractor has full knowledge of the extent and character of the contemplate work and any problem to be encountered in the performing same. II. Project Description Scope of Project: To provide driver's education for students in order to receive their New Hampshire Driver's License. III Specific Requirements: Contractor: The Subcontractor agrees to provide students of the New Hampshire Job Corps Center with 40 hours per week of the services stated herein for a one year. The service period for this subcontract is October 1, 2015 through March 31, 2016. Service periods are subject to change upon contract awards. Such services will be provided as authorized by the Center Director or his/her representative as follows: 1. Provide Driver Education to include classroom and road training for New Hampshire Job Corps Center students in accordance with the curriculum and requirements of the New Hampshire Department of Motor Vehicles. 2. Ensure that instruction is in accordance with the requirements of the USDOL contract, the Policy and Requirements Handbook (PRH), and Adams and Associates Standard Operating Procedures and handbooks, utilizing Job Corps developed curriculum and Program Instruction and Training Achievement Records. 3. Ensure that instruction incorporates a process of self-paced, open-entry/open-exit format learning and is structured in three (3) modules: Module 1, Classwork; Module 2, Driving Instruction; Module 3, On-the-Road practice. 4. Ensure that students complete a minimum of thirty (30) hour of classroom instructions with certified drive education instructor, ten (10) hours of behind-the-wheel practice with a certified driver education instructor, six (6) hours of driving observation with a certified driver education instructor, and are, in the instructor's opinion, competent to be scheduled for a road test. 5. Schedule and transport students to the Registry of Motor Vehicles testing centers as appropriate. 6. Maintain accurately all required student progress records and other student reporting documentation as requested by Adams and Associates. 7. Ensure that the interaction between the instructor and the students meets all of the required standards as set out in the documentation shown in paragraph two (2) above. 8. The instructor will not be required to perform services on those weeks that the students are on break (usually 4 weeks per year) or as directed by Adams and Associates in the monthly schedule. Adams and Associates, Inc. through the New Hampshire Job Corps Center shall provide: 1. Equipped workrooms and training facilities at the New Hampshire Job Corps Center to be utilized by the instructor. 2. A Driver Education vehicle, equipped with dual brakes, and prominently identified as a Driver Education vehicle. 3. Registration as a Driving School 4. Orientation for the instructor in all New Hampshire Job Corps Center procedures and requirements. 5. Staff support for the instructor in the administration, supervision, and counseling of students. 6. By the 1st day of every month, a schedule for the instructor showing the hours and days of the following month that the instructor is required to provide Driver's Education Services. 7. Payment to the instructor for services provided. 8. All necessary documentation to support the instructor's role as a service provider at the New Hampshire Job Corps Center. 9. A clear reporting process for the instructor to deal with administrative and student concerns and issues that arise in the context of this service. 10. Insurance coverage for all students undertaking Driver Education training and for all of the vehicles being used for that training. 11. Payment of all student fees for Learner's Permits, Tests, and Driver's Licenses. Note: Please check in at the welcome center on the scheduled days of service. IV Center Requirements Center Regulations: Contractor and his employees shall abide by all of the Center's rules and regulations. There shall be no fraternization between contractor personnel and Job Corps Center students or personnel. The Contractor will be required to follow all Center safety and security requirements. Management of Jobsite: The Contractor shall maintain a clean and safe workplace that is secured at the end of each working day. Insurance: The Contractor shall obtain and maintain insurance as follows: Workman's Compensation/Employers Liability insurance, to meet New Hampshire statutory requirements Comprehensive General Liability ($1,000,000 per occurrence) Comprehensive Automobile Liability ($1,000,000/combined single limit) Fire Legal Liability ($500,000 per occurrence) The Contractor must cover any deductible costs. Upon contract award, the Contractor must add New Hampshire Job Corps Center as additional named insured to the policies. Schedule: From October 1, 2015 to March 31, 2016. V. Proposal Form The Contractor's proposal must be submitted on company letterhead, must reference the proposal ID # NH2015-0001 and address all items in sections II & III, to be considered. Bids must to be valid for 90 days. The Contractor shall submit the following information with the proposal: References Copies of Insurance Certificates and applicable licenses. Completed vendor questionnaire and W9. All questions must be in writing and sent to the Purchasing Agent Rodriguez.Ruber@jobcorps.org No Phone calls will be accepted. A. Bids can be mailed to Ruber Rodriguez, Purchasing Agent. 1. A minimum 90-day bid guarantee is required. 2. This project is tax-exempt. 3. Prior to starting any work, the contractor shall show proof of required insurance, in amounts to cover risk as stated below: • Comprehensive General Liability Insurance ( $1,000,000 combined single limit) • Comprehensive Automobile Liability Insurance ( $1,000,000 combined single limit) • Workers Compensation / Liability Insurance ($1,000,000 per occurrence) • Fire Legal Liability of at least $500,000 per occurrence. 4. Final Bid Proposal shall be typed and submitted on official Company letterhead and must include the following: • Project ID#, Project Description, Project Specific Requirements, Lump Sum Cost • A list of materials (Eco-sensitive materials should be identified) to include a list of any reused parts, equipment or conduit, and material safety data sheets (MSDS) proposed for use, if any • References • Copies of all Insurance Certificates • Copy of all licenses and certifications relevant to this proposal • Proof of a valid Contractor's License for the State of New York • Completed Vendor Questionnaire (with DUNS# if available) and W9 • Small Business Certification, if any 5. Net 30 payment terms
