Description
Nuclear Escape Respirators SAQMMA-15-I-0041 1.0 Purpose This is a Request for Information (RFI) as) Pursuant to FAR 52.215-3, Request for Information or Solicitation for Planning Purposes (Oct 1997). The purpose of this RFI is to accomplish market research pursuant to Federal Acquisition Regulation (FAR) Part 10, and to identify qualified businesses and interested sources capable of providing the supplies described herein. This notice is issued solely for information and planning purposes and does not constitute a Request for Proposal (RFP), Invitation for Bid (IFB), Request for Quote (RFQ) or any other type of commitment on the part of the government to conduct an acquisition for the below-listed supplies in the future. Respondents are advised that the Government will not pay for information submitted in response to this RFI, nor will it compensate respondents for any costs incurred in the development/furnishing of a response. The Government will not entertain telephone calls related to this RFI. Please note that a decision not to submit a response to this RFI will not preclude a vendor from participating in any future RFP, IFB or RFQ. Note: Interested vendors should be registered in the System for Awards Management (SAM). 2.0 Background The Bureau of Diplomatic Security (DS) is the law enforcement and security arm of the Department of State (DOS). DS is responsible for providing a safe and secure environment for the conduct of U.S. foreign policy. The mission of the WMD Countermeasures Division within DS is to design, implement, and sustain a global WMD Countermeasures Program that provides posts worldwide the necessary knowledge, equipment, and training to respond to a chemical, biological, radiological or radiological attack. 3.0 Requirements The WMD Countermeasures Division is in search of potential sources for NIOSH certified Chemical, Biological, Radiological and Nuclear escape respirators in accordance with 42 CFR 84, which is the standard for occupational safety and health products. The respirator must protect the user against the inhalation of chemical, biological, radiological, and nuclear particulate matter for a minimum of 15 minutes, 30 minutes preferred. The respirator shall allow for easy facial recognition, and clear verbal communication and use of communication devices (i.e., phones). 4.0 Instructions If your firm is interested in this opportunity, the Government requests that your firm respond to this notice with the following information: 1.Company name, address, points of contact including phone numbers and e-mail addresses, manufacturing sites and locations; 2.In an easily identifiable location, include your company cage code and DUNS Number, as well as the socioeconomic status under NAICS Code 339113 (e.g. large, small business, Hubzone, 8(a), woman-owned, etc) or other relevant and appropriate NAICS codes; 3.Past Performance references along with brief descriptions of the respirators manufactured for the past three (3) years 4.Any certifications, special licensing or industry recognized awards or credentials your firm may hold regarding the manufacturing of the respirator identified in your firm's response; 5.Please provide details of your firm's manufacturing approach, methods, standards, management, quality/inspection along with other related information; 6.Include detailed information of the specifications of the respirator (please include pictures), indentify sizes, if the mask is manufactured in the U.S. or abroad/where, storage and operating temperatures, if the respirator provides protection from carbon monoxide and smoke, CO2 concentration inside respirator during use, detailed identification of operability, any Material Safety Data Sheets (MSDS) for the respirator or its components, and if any additional tools are needed to operate the device; 7.Identify how each respirator is labeled (i.e., serial number, lot number, manufacture date and expiration date) and identify how units are packaged, including the materials used for packaging; 8.Include any training manual (indentify whether or not a copy of the manual is in each packaged unit[s]) and discuss any additional training that is needed to operate the respirator; 9.Include warranty information and service life when retained in original packaging, as well as limits in product function if not used by a certain date post-manufacturing; 10.Identify any positive or negative environmental factors, whether in the manufacturing or use of the respirator; 11.Include the logistical operation approach: Is your firm able to deliver respirator from sites, and what is your firm's customary procedure for transporting respirators? 12.Discuss customary delivery timelines for respirators for bulk orders (assume 10,000 units), and what is the typical timeline from manufacturing to final inspection/delivery; 13.Respondents must indicate whether they offer the required products on ANY Federal Supply Schedules.
